Program Management: • Monitoring the quality and effectiveness of the mental health services provided to youth, • Plan and oversee daily STRTP program schedule, including therapeutic groups, outings, and activities., • Ensure program fidelity and quality outcomes for clients., • Monitor client progress and coordinate care with interdisciplinary teams., • Lead program development initiatives and participate in quality improvement activities., • Collaborate with the STRTP Administrator to ensure smooth program operations without overstepping administrative authority., • Coordinate, assign, and complete the daily clinical shift exchange to ensure accurate communication of client updates, treatment needs, and clinical priorities., • Participate in the intake process, including reviewing referrals and assessing appropriateness for placement., • Support and oversee discharge planning and transition services., • Participate in licensing inspections, county audits, and program reviews., • Assist in developing and updating program policies, procedures, and clinical protocols., • Participate in the leadership on-call rotation, providing phone support and responding in person when needed to ensure safety and required staffing levels. Staff Training & Development: • Develop and implement training programs for all clinical and direct care staff., • Ensure staff are trained in evidence-based practices, regulatory requirements, and trauma-informed care., • Monitor staff adherence to treatment protocols and program policies. Collaboration & Community Engagement: • Work closely with psychiatrists, psychiatric nurse practitioners, and other mental health professionals., • Participate in Child and Family Team (CFT) and interdisciplinary team meetings., • Foster positive relationships with placing agencies, social workers, probation officers, families, and community resources., • Cultivating effective partnerships with applicable county offices, school districts, law enforcement agencies, emergency services, and other community providers, and facilitating effective training for partners, as requested. POSITION DESCRIPTION The School Nurse at La Cheim School delivers trauma-informed, evidence-based medical and mental health services in a non-traditional school setting for youth with emotional and behavioral challenges. The nurse integrates psychiatric-mental health and school nursing practices to promote student wellness, support crisis stabilization, and facilitate academic engagement. The School Nurse provides direct services including individual health assessments, medication administration, care coordination, psychoeducation, classroom observations, and participation in multidisciplinary meetings. ESSENTIAL DUTIES • Conduct health assessments, screenings, and develop care plans., • Administer medications, manage refills, and maintain accurate MAR documentation., • Address urgent medical needs and make appropriate referrals within Contra Costa County and Alameda County., • Maintain updated records and complete Medi-Cal billing progress notes within 3 days of service., • Monitor for signs of physical or emotional distress and initiate timely interventions., • Participate in crisis response using evidence-based de-escalation techniques., • Engage with students across the school day, including classroom observation, elective, and lunch., • Support students in emotional regulation and psychosocial rehabilitation activities., • Actively participate in IEP and Child and Family Team meetings, contributing nursing expertise to treatment plans. Engage with outside providers of youth's treatment team, • Coordinate care with internal teams, guardians, juvenile hall, hospitals, and outside providers., • Ensure HIPAA-compliant communication and maintain professional boundaries., • Lead monthly health education classes for students., • Train staff on health and safety procedures within the nurses scope of practice., • Promote awareness of chronic illness management, hygiene, medication adherence, and emotional wellness., • Maintain an audit-ready, safe, and stocked nursing station., • Ensure compliance with infection control policies, medication handling regulations, and confidentiality standards., • Assist with CPS reports when needed., • Collaborate with school Psychologist to schedule weekly on site psychiatrist appointments., • Secure consents, medical records, and documentation from guardians or court-appointed representatives.
