Job Description EMPLOYER: Pediatric Nursing Specialists of Indiana, a Home Care Network (HCN) agency. JOB TITLE: RN Case Manager (PRN / As Needed) • This role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews., • At least 1 year of Case Management experience and home health agency (or equivalent) experience are required., • Must have experience with OASIS and NON-OASIS assessments., • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus., • Flexible and customizable. As a PRN RNCM with HCN, we'll work around your other employment and/or personal schedules, but will require most of your availability to be Weekdays, Daytime., • We prioritize visits as close to your home as possible. - that fits your availability and preferences., • No On-Call requirements / No Overnight shifts required., • Weekend, Overtime, Respite & other Nursing PRN services are offered when available but are not required to accept. To support our growth and the increasing volume of client care throughout Indiana, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (PRN / As Needed) to join our agency and serve existing and returning clients in the greater Indianapolis and Metro areas. • This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). Responsibilities/Requirements of an RN Case Manager / Clinical Nurse Supervisor include: • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations., • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides., • RN to Document/Communicate/Coordinate patient status/change in condition to care team., • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers., • Willing to travel surrounding counties., • Graduate of an accredited School of Nursing., • Minimum of one (1) year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Indiana., • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience. 1+ years of paid home health / in-home care experience is also required. 2+ years of both are preferred., • Minimum of one (1) year Experience in case management is required., • CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties)., • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus., • Strong background in nursing, patient care, and confidentiality., • Strong attendance and dependability track record with previous employers., • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive., • Familiarity with HIPAA regulations and medical terminology., • Current Driver's License and reliable transportation.
