The Peer Specialist will perform a wide range of tasks to assist peers in regaining independence within the community and mastery over their own recovery process. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. Job Description The Mental Health Association is seeking Per Diem Peer Support Specialist for our new program, 988 Mobile Crisis Outreach Team, operating out of Hudson county. The 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. POSITION RESPONSIBILITIES: • Provide a comprehensive case management assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family strengths/needs;, • Develop an Integrated Family Case Plan that is person-centered and includes strategies for recovery. The plan shall identify priorities, desired outcomes and the STAR - 8 strategies and resources to be used in obtaining outcomes based on the case management assessment;, • Link to appropriate care and resources in the community including resources that address specialized needs, such as agencies providing services related to HIV/AIDS, mental health disorders, chronic and acute health problems and problems stemming from involvement with the criminal justice system;, • Promote the STAR individual’s recovery to his/her fullest potential by facilitating assessment of skills, including literacy, educational opportunities (GED, certification programs), job readiness skills, vocational training, employment placement and retention and career development;, • The STAR case managers are required and must document that each individual was offered a visit minimally one (1) hour per week by a staff member, whether that visit was accepted and document their participation in the visit and any services received. PRIMARY RESPONSIBILITIES • As part of an interdisciplinary team the Peer Specialist will be responsible for supporting a caseload of 15–20 high-needs (serious mental illness, substance use, homelessness) participants at a time, providing face-to-face, telephonic, and community Peer recovery support and advocacy., • Establish supportive, peer-based relationships grounded in trust, mutual respect, and shared experience, • Provide one-on-one peer mentoring, advocacy, and emotional support to program participants, • Assist participants in identifying goals related to housing, employment, wellness, sobriety, and social connection, • Accompany participants to court appearances, community appointments, or service referrals as needed, • Support participants in navigating court requirements, probation/parole expectations, and community service obligations, • Provide court-based Peer support to participants upon completion of arraignment, identifying immediate needs or concerns, and facilitating immediate connections to community-based services, • Facilitate or co-facilitate peer-led groups focused on recovery, emotional regulation, life skills, and reentry planning, • Collaborate with case managers and legal advocates to ensure participant needs and preferences are understood and honored, • Participate in regular supervision, peer development meetings, and trainings related to harm reduction, trauma-informed care, and justice-system navigation, • Document participant engagement and progress in a timely manner in an electronic case management system in accordance with program guidelines and confidentiality requirements, • Uphold ethical guidelines, competencies, and boundaries appropriate to peer support roles REQUIRED QUALIFICATIONS: • HS Diploma or equivalency required, • NYS Peer Specialist certification (CPS) OR Recovery Peer Advocate certification (CRPA) required. Certification must be maintained and kept current while employed in this position., • 2 years of professional experience as a Peer Specialist, Peer Advocate, Peer Navigator, or Peer Counselor in a Behavioral Health or Criminal Justice setting required, • Personal lived experience with the criminal legal system and/or recovery from substance use, mental health, or homelessness, • Ability to model recovery-oriented practices, • Strong communication and interpersonal skills, • Ability to maintain boundaries and practice cultural humility, • Familiarity with community-based resources including housing, treatment, employment services, and legal supports, • Proficiency in using computer software applications such as Microsoft Office suite (Word, Excel, PowerPoint, Outlook), email, web browser and database/file management systems required, • Reliable transportation and ability to travel locally as needed PREFERRED QUALIFICATIONS: • Experience working or volunteering in peer support, reentry programs, harm reduction, or behavioral health settings, • Knowledge of trauma-informed and restorative justice principles, • Bilingual or multilingual (especially Spanish, if applicable to your region) WORKING CONDITIONS: • This role involves a combination of office, community, and courtroom settings, • Occasional evening or weekend hours may be required, • Peer Support Specialists must be able to model wellness and uphold recovery principles in challenging environments COMMITMENT TO DIVERSITY, EQUITY, INCLUSION AND BELONGING The New York City Criminal Justice Agency is committed to building a team that reflects the communities we serve. As part of a multidisciplinary, core services team, the Wellness Clinician-Licensed provides assessment and direct clinical services to people who are experiencing a mental health crisis or a worsening of a pre-existing mental health disorder or a co-occurring disorder who are enrolled in the CIS Program. Essential Job Functions Create and Maintain: • A wellness and recovery-oriented environment by using person-first language, respectful documentation, and person-centered planning. All services provided will promote a wellness and recovery outcome, helping to stabilize an individual so they may remain in the community, and divert from having to go to the Emergency Room., • A culture of compliance with internal and external policies, regulations, laws and high ethical standards Provides: • Under the clinical supervision of Senior Wellness Clinician conducts intake assessment, DSM-V diagnosis, goal planning, brief treatment, crisis stabilization, linkage, and discharge planning services for individuals with mental health and co-occurring disorders, inclusive of substance abuse and medical disorders. As part of this, a Risk Assessment is conducted with each individual to assess suicidal and/or homicidal ideation with clinically indicated follow up if needed., • Under the clinical supervision of Senior Wellness Clinician provides mental health and substance abuse treatment and education on-site to persons served and significant others, of ten focusing on evidence-based, trauma informed brief treatment., • Under the clinical supervision of Senior Wellness Clinician provides crisis intervention services for persons served with acute psychiatric needs whose circumstances and conditions require rapid intervention. Interventions are varied, and include but are not limited to the following: Motivational Interviewing; Cognitive Behavioral Treatment; Crisis Stabilization and De-Escalation; Trauma Informed Care; Self-Regulation; Psychoeducation; and other interventions that may be clinically indicated., • Under the clinical supervision of Senior Wellness Clinician provides brief therapy and evidence-based interventions in individual or group sessions in the office., • Facilitates Involvement of family members at person served request to provide psychoeducation and support, • Technical assistance and education to members of the team in reference to best clinical practices. Utilized best practices materials and strategies for individual and group intervention in conjunction with the team, • Collaboration with Wellness Specialists to recommend needed case management services along with referrals and linkages to after care services. Work collaboratively with APN’s and Psychiatrist in providing Medication Education to persons served., • 24-hour on-call coverage on a rotating schedule among other Licensed Clinicians along with Senior Wellness Clinician and Director. Clinician’s on-call will determine acuity of the individual’s situation, and will contact the police or Mobile Crisis as indicated. If the individual is stable, the clinician will provide information about CIS Hours of Operation and encourage the individual to come in to receive services during those hours., • Under the clinical supervision of Senior Wellness Clinician provides Crisis Intervention, Engagement, Needs and Strengths Comprehensive Assessment to include diagnosing, Collaborative Goal/Discharge Planning, Brief Therapy, Individual and Family Psycho-education, collaborative documentation, and Trauma Informed Care. Coordinates: • Intake, assessment, Goal, and Discharge planning to include the CIS staff. This is achieved through regular multi-disciplinary communication between all team members, inclusive of other Licensed Clinicians, Wellness Specialists, APN’s, Psychiatrist, scheduling & Insurance Verification Specialist and Receptionist, and Program Director. Responsible: • For maintaining positive relationships with community service personnel, • For ensuring that intake processes are readily accessible and flexible and that all individuals who either walk in or are referred are seen and assessed within 24 hours by a Licensed Clinician., • For ensuring that clinical interventions are evidence based and lead to goal attainment within the framework of the BCIS programs 30-day period for persons receiving services. Participates: • Within the team, in the development of comprehensive assessment and the Goal and Discharge Plan, focusing on clinical needs of the person receiving services., • In team Meetings, held on a weekly basis, where persons served are reviewed as needed and other clinical and operational matters are discussed., • Participates in required weekly Individual Clinical Supervision with Senior Wellness Clinician. Consults: • Community service providers and resource providers regarding the assessment, treatment and aftercare needs of the persons served.
