Job Description POSITION SUMMARY: The Nurse Coordinator will perform the duties required to coordinate and monitor the provision of Substance use care including medication for patients receiving outpatient treatment for substance abuse. The Nurse Coordinator will also perform the duties of a Clinic Nurse Coordinator in providing medical care and coordination services for all patients. POSITION QUALIFICATION REQUIREMENTS Education: Bachelor of Science Degree in Nursing preferred. Experience: • Three (3) years' experience in primary care, behavioral health, community health, or related field., • One (1) years' experience in substance abuse or addiction environment (preferred)., • Three (3) years' experience in case management (preferred). Skills and Abilities which may be representative but not all inclusive of those commonly associated with this position: • Good computer skills including the use of Microsoft Word, Excel, and Outlook., • Solid experience using an electronic health record., • Excellent oral and written communication skills., • Detailed oriented and strong organization skills., • Good analytical, critical thinking and problem-solving skills., • Able to work on multiple tasks simultaneously., • Knowledge of crisis intervention, • Ability to work independently, as well as with a team., • Ability to motivate and educate clients and staff. Residency and Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri. License(s)/Certification(s) Required: • Registered nurse licensed to practice in the State of Missouri, • CPR Certification FUNCTIONS OF THE JOB Essential Functions • Performs initial medical triage on patients requesting substance use services., • Assesses patients 'physical/substance use disorder status, analyzes data, and integrates assessment resulting in planning for the care of the outpatient therapy for opioid and alcohol use disorder therapy., • Demonstrates critical thinking skills to achieve outcomes and solve problems., • Coordinates the management and administration of medications prescribed by the medical clinician including inventory, ordering, compliance, and vendor agreements., • Performs medication authorization and refills., • Performs urine drug screening and monitoring and provides all reports to the clinician and team members., • Observe sign and symptoms of medical issues, relapse, documents, and reports reactions of the treatment to the provider., • Delivers organized care by prioritizing care needs and tasks effectively. Adapts care to changes in patient's condition., • Communicate with patients, team members, in an accurate, timely, and professional manner., • Provides health education and counsel patients and family members in medication treatment, substance abuse., • Collaborates with multidisciplinary team and coordinates further substance abuse services/mental health treatment as needed., • Utilizes evidenced based practices to solve problems., • Maintains the integrity of patient medical records, completes appropriate forms and reports accurately, and adheres to patient confidentiality., • Performs other duties as assigned. SUD Duties • Monitors SUD phone calls., • SUD intake and triage with appointment scheduling to appropriate medical clinician., • MOUD patient education regarding Narcan, harm re-education, MOUD process and communication (same for alcohol)., • Coordination of care with LCSW and clinician., • Assistance with prior authorization or paperwork for injectables., • Administration and tracking of grant support such as bus passes, medication coverage., • Tracking MOUD follow-up calling for missed appointments., • Quality tracking of SUD referrals (internal/external), initiation, continuation, lost to following up reason., • Quality Coordination with quality manager and DRVS., • Grant reporting with focus on the clinical and quality portions., • Tracking and administering injectable medications for SUD (majority opioid and alcohol)., • Assistance with updating marketing/flyers for the program with Communication Manager.
