Work Environment\n This position is remote, offering you the flexibility to work from anywhere while primarily serving clients connected to our detox center facility. You will enjoy a standard full-time schedule from Monday to Friday, 8:00 AM to 4:30 PM, while also being available for occasional evening or weekend hours as necessary to meet client needs. This role requires adaptability in scheduling and may involve being on-call to address urgent situations, ensuring that you can provide continuous care and support. Key Responsibilities\n\n • Conduct thorough psychosocial assessments to evaluate the mental and emotional needs of seniors, factoring in their social circumstances.\n, • Develop tailored care plans in collaboration with clients and their families, outlining specific goals, preferences, and interventions.\n, • Provide counseling and emotional support, helping seniors navigate life transitions and cope with various challenges.\n, • Advocate for clients to ensure they have access to essential community resources and receive appropriate support and services.\n, • Collaborate with an interdisciplinary team comprised of healthcare professionals, nurses, and caregivers to coordinate and enhance client care.\n, • Monitor clients' well-being through regular visits to assess living conditions and address any safety concerns.\n, • Facilitate communication among family members regarding care decisions, ensuring alignment and understanding across involved parties.\n, • Educate clients and families about community resources, support groups, and available programs to enhance their social engagement and overall quality of life.\n, • Maintain detailed documentation of client interactions, assessments, care plans, and progress notes, adhering to best practices in record-keeping.\n, • Stay informed on current social work practices, policies, and ethical standards to enhance service delivery and compliance.\nPatient Population / Client Focus\n You will primarily provide services to senior clients, many of whom may be experiencing significant life changes or challenges related to aging, such as health declines, loss of loved ones, or difficulties with daily living activities. Your expertise as an LMSW will be crucial in understanding and addressing the complex psychosocial needs of this demographic, promoting their well-being and ensuring they maintain a high quality of life.
