About the Role The Transgender Health & Wellness Program Coordinator leads the implementation of innovative health and wellness initiatives designed to improve the lives of transgender, gender non-conforming, and non-binary community members. This role combines program leadership, community outreach, direct services, health education, HIV prevention, advocacy, and partnership building to ensure affirming, culturally responsive care reaches those who need it most. Working closely with the Director of Interventions, you'll coordinate programs that connect individuals to healthcare, prevention services, mental health supports, housing resources, and community while helping shape a healthier and more equitable future. What You'll Do Lead Community Health Programs • Coordinate HIV, STI, Hepatitis C (HCV), and overall health and wellness programming., • Implement evidence-based and community-informed prevention interventions., • Provide HIV/HCV testing, linkage to care, prevention education, patient navigation, and case management., • Ensure all services are trauma-informed, culturally affirming, and grounded in harm reduction. Build Relationships That Matter • Develop trusted relationships throughout transgender and gender-diverse communities., • Lead outreach efforts in community spaces, healthcare settings, and local events., • Plan wellness events, support groups, educational workshops, and community engagement activities., • Build partnerships with healthcare providers, community organizations, colleges, businesses, and government agencies. Support Health Equity • Connect participants with affirming healthcare providers and social services., • Help reduce barriers to healthcare access., • Promote HIV prevention strategies including PrEP, PEP, routine testing, and treatment., • Advocate for equitable, culturally responsive care for TGNCNB communities. Ensure Program Excellence • Monitor program outcomes and maintain accurate documentation., • Prepare reports for federal, state, and private funders., • Ensure compliance with HIPAA, New York State HIV Confidentiality Law, and grant requirements., • Conduct quality assurance and continuous program improvement. Support Team Success • Assist with training, onboarding, mentoring, and supporting program staff and volunteers., • Foster a collaborative, respectful, and mission-driven team environment., • Work across departments to strengthen client services and organizational impact. What You Bring Required Qualifications • Bachelor's degree in Public Health, Social Work, Human Services, Community Health, or a related field., • Minimum two (2) years of experience in HIV prevention, LGBTQ+ health, public health, case management, or community outreach., • Strong knowledge of transgender health, LGBTQ+ communities, HIV prevention, and harm reduction., • Experience facilitating outreach, education, and community engagement., • Excellent communication, relationship-building, and organizational skills., • Ability to work occasional evenings and weekends., • Valid driver's license and reliable transportation. Preferred Qualifications • HIV Testing Certification (or willingness to obtain within 90 days)., • Experience supervising staff, interns, or volunteers., • Bilingual (Spanish or another language)., • Experience with New York State Department of Health or other grant-funded public health programs. Salary $49,500–$54,900 annually Salary is commensurate with experience, qualifications, education, and demonstrated expertise in transgender health, HIV/STI/HCV prevention, community outreach, case management, and program coordination.
