The program will provide comprehensive, intensive early intervention services for youth exhibiting high or at-risk behaviors. The programming builds upon youths’ resiliency and sense of self-worth for positive outcomes and redirection. There are no traditional office hours. Case Managers will work primarily in the afternoon and evenings, specifically during the school year and at weekends to provide maximum involvement with the family and the identified clients. There may need to be a need to start early to help a participant get to school. The Case Manager will be placed on an on-call roster and will be responsible for providing emergency assistance to clients assigned in the program 24 hours a day while on duty. POSITION RESPONSIBILITIES:, • IPS is a program designed to provide a safe and holistic environment for at-risk youth to develop themselves and their confidence, through an engaging, supportive and strength-based environment. IPS staff will develop programs and activities that include the 7 core concepts of resilience, which are the program’s core values., • The major responsibility, therefore, is to resolve issues, ensure stability and to develop positive youth development and academic support., • The IPS Case Manager is responsible for providing service linkages for all members of the family as appropriate., • The IPS Case Manager will facilitate all academic progress in collaboration with the youth’s school., • The Case Manager will collect all academic records to determine current academic status of youth and make personal contact with school counselor and teachers to gather all necessary educational information including attendance, behavior reports and Statewide testing reports., • The Case Manager will develop an academic support action plan with the school counselor thorough n inter-agency meeting with the goal of improving and supporting the youth academic progress, • Coordinate and monitor the implementation of the academic support action plan., • Engage care givers to take a more active role in the youth’s school settings by accompanying them to school meetings and attending school functions., • Assist the parent/caregiver to advocate for their child’s educational needs and helping them navigate through the school support system and introducing them to the Educational Law Center and other educational advocacy groups and parent groups., • Coordinate with school counselor to develop post-secondary plan for older youth., • Ensure prior to and during the discharge planning the enrollment in after school or other remedial or enrichment activities as needed, especially beyond the youth’s involvement in the IPS program., • The IPS Case Manager will co-facilitate the site based program in which each youth will participate three times a week., • The main tenets of the role include identification of, and referral to, needed services, agency leadership in coordination, and active goal-directed support for the youth and family., • The Case Manager will work collaboratively with the assigned counselor and develop activities and coordinate community service/engagement programs. At crisis times, there may be a need to “double team”., • The maximum caseload is 17 clients. Case longevity is determined by court and or DHS personnel., • The Case Manager’s role is to be able to report to the court that identified issues have been resolved through a collaborative effort with school personnel and community resources and through the site-based programming activities., • Case Manager must meet with the supervisor weekly for supervision., • Case Manager must meet with the family and see all children once each week, and must coordinate work with others involved with the family, possibly including parent aides. Minimum DHS contact stipulations must be adhered to., • Case Manager will form an “after hours on call” roster, with alternating back up of the program director. In this way, families can access our staff round-the-clock., • Case Manager will ensure that services and counseling are commensurate with identified issues, and may need to be increased beyond minimum standards., • Following assessment of family’s identified needs, Case Manager will identify community resources, and work collaboratively with the Counselor to assist families with linking them to the appropriate community resources., • Case Manager is responsible for ensuring that school age children receive the appropriate services that meet their educational needs., • Case Manager will ensure that he/she keeps accurate documentation specifically all unsuccessful efforts to maintain the expected level of intensity and monitoring. All documentation and communication will be directed to the assigned DHS worker or court personnel via e-mail correspondence, telephone calls, and with hard copies to be kept in client’s file., • Case Manager will facilitate the family’s transportation for appointments with other service providers by providing directions, tokens, as needed, and accompanying on public transportation or driving them to appointments and meetings in their own vehicles., • Case Manager must attend monthly team sessions to determine and monitor service appropriateness for each youth and to inform the team of the client’s adherence to orders of the Court and to the program., • Case Manager must attend all mandatory IPS trainings and activities hosted by the Department of Human Services or designee., • Participates in meetings to ensure priorities are clear, coordination is good and communications are open., • Cooperates and communicates as a multi-discipline staff team member through formal meetings, informal discussion and other participation as necessary., • Models traits of a responsible team member, executing job responsibilities, open communication, good follow-through; supports team members to do the same. Job Description SALARY-$20/hr. HOURS: SATURDAYS ONLY- 9 AM-4:30 PM GENERAL OVERVIEW OF KEY ROLES & RESPONSIBILITIES: 1. The individual occupying this position will provide supervised visits and parent exchanges while allowing the child to interact with the non-custodial parent in a safe environment, and to promote a healthy relationship when possible. The primary purpose of the visit is to allow children to preserve relationships with people who are important to them under the guidance of the Family Court. 2. To monitor court-ordered visits and parent exchange by overseeing the interactions between children and the non-custodial parent so that the child/family benefits as much as possible from the contact. 3. To assist children and their families in maintaining or reestablishing relationships that are healthy and safe in a neutral space. 4. To provide custody evaluators with accurate documentation and supplemental information as needed on the visitation and parent exchange process. ESSENTIAL & CORE FUNCTIONS: 1. Job Description Position Title: TIPS Program Supervisor Reports to: TIPS Program Director Department: Administration Adoption Behavior Health HBDS IPS PEP Face Forward Foster Care Residential TIPS Program: FLSA Status: Exempt Non-exempt Location: 100 W. Coulter Street POSITION SUMMARY: The Truancy Intervention and Prevention Services (TIPS) Supervisor is an integral member of the TIPS program. The Program Supervisor reports to the TIPS Director and deputizes on their behalf during absences. The TIPS Supervisor must communicate all concerns and issues upwards to the TIPS Director and in their absence to the Executive Director. The TIPS Supervisor will work with TIPS Director and a team of Truancy Case Managers that are assigned to specific schools in the Northwest section of the City of Philadelphia. The program works in collaboration with the Philadelphia School District and the Philadelphia’s Department of Human Services (DHS), Community Umbrella Agencies (CUA), and other stakeholders with the mission to help children and youth improve their school attendance and address the issues that are impacting their ability to attain satisfactory school attendance and school achievement. EDUCATION/CREDENTIALS/ EXPERIENCE: A Master Degree in Social Work or related field with at least two years of supervisory and specific social service work experience. (I.e. related fields are social services, sociology, psychology, theology, education, and public administration) is required. Staff supervision is subject to review by DHS., • The TIPS Supervisor will lead the Truancy Case Managers in resolving issues and barriers that families face and help eliminate the barriers by providing intervention and advocacy services., • The TIPS Supervisor is responsible in ensuring that the comprehensive plan is developed in collaboration with the youth, parent, and others involved with the case, and that community linkages are in place prior to the case being discharged., • The TIPS Supervisor must attend all truancy meetings and trainings., • The TIPS Supervisor will monitor updates and complete a detailed CBPS database report every two weeks to ensure DHS & Agency Compliance., • The TIPS Supervisor is responsible in completing a detailed file review report of each case manager’s case load, every month to ensure DHS & Agency Compliance., • The TIPS Supervisor is responsible to ensure that all TIPS responsibilities regarding court are maintained –court reports reviewed and signed, and the timely submission of court reports to DHS Liaison., • The TIPS Supervisor is responsible to ensure that all CM’s to appear in Regional Court by no later than 8:30am with their detailed court sheet completed with the TCM Supervisor’s signature (as requested in court services section) to submit for the Hearing Officer to review. Coulter Street POSITION SUMMARY The IHD Caseworker provides casework and supervision to clients and families in the In-Home Detention component of the Home-Based Delinquent Services Program. Cases from other HBDS components (In-Home Detention) may be allocated on an as needed basis. The In-Home Detention Case Worker reports directly to the Supervising Social Worker or Director of the IHD program. The position is Fee for Service and cases are allocated on an as needed basis. Cases are not guaranteed, and the position is a contracted position, therefore, hours, assignments and cases fluctuate week to week. The actual daily hours and workdays are determined between the individual IHD case worker and the supervising social worker, in order to ensure daily coverage (7 days a week). POSITION RESPONSIBILITIES • IHD casework and monitoring., • The IHD Caseworker provides monitoring and supervision of 5 contacts per youth per week in the IHD program up to 6 cases. All contacts must be documented and submitted for review on a weekly basis. Contacts consist of visits made by the IHD Caseworker to meet with the youth face to face for a brief contact or transporting a youth on an activity with other youth where IHD Caseworkers conduct educational, life-skills counseling, recreational, cultural and social activities that may benefit the client., • The IHD Caseworker meets the objectives of providing sufficient supervision to ensure the youth's appearance in court, the client’s remaining arrest-free, the observation of curfews, and school and/ or work attendance., • When a client referral is received from the JJC, the first face-to-face contact must be made within 24 hours., • At the initial meeting with the youth and the family in their home, the IHD Caseworker describes the program requirements (curfews, school attendance, and making court dates). Required forms are completed and a schedule worked out for the coming week., • The IHD Caseworker makes clear to youth and family that he/she is required to report immediately to the JJC IHD Caseworker the following., • His/her arrest, • An absence without permission, • Curfew violation, • Unexcused absence from school, • Relocation without permission, • Serious family breakdown, • Missed court date, • Any serious incident that involves police, fire, or medical attention, • The IHD Caseworker must ensure court dates are kept and that hearing results are communicated to the program director., • The IHD Caseworker ensures the youth's court appearance by picking up the client and family at their home and taking them to the scheduled hearings., • The IHD Caseworker provides written court reports for court., • Upon arrival at court (usually by 9:00 a.m.) the IHD Caseworker checks the youth in with the court clerk. He/she/they are also responsible for making contact with the client’s attorney, usually a Public Defender, explaining that the client is committed to the IHD program., • The IHD Caseworker is required to turn in proper court reports, progress reports and any other reports to the court and attorneys concerning the client’s adjustment and or progress., • The IHD Caseworker accompanies the youth and family into the court room and must be prepared to make an oral report and to answer any questions the Judge may have., • If the case is continued, the IHD Caseworker must specifically ask the Judge or clerk if the youth is still committed to the In-Home Detention program., • The IHD Caseworker is responsible for explaining the court outcome to youth and family., • The IHD Caseworker is responsible for maintaining the youth’s record in accord with JJC policies and procedures. SPECIFIC DUTIES: Client Record Management/Compliance • Assure compliance with all regulations, most notably Chapter 6400 and Chapter 6100 as they pertain to fiscal requirements for individuals served in our residential program., • Maintain a current and necessary fiscal file, including but not limited to the Room and Board Agreement, Social Security Administration award letters, medical assistance required documentation and all necessary supporting financial documentation including bank statements and income/expenditure transactions for annual recertification and licensing., • Maintain tracking system to review and analysis Residential Financial Files monthly for accuracy and any fiscal irregularities are resolved and report any fiscal irregularities to your supervisor., • In accordance with 6100 regulations in coordination with the CLA department complete a Supplemental Security Income (SSI) application and support the appeal to assure compliance with regulations., • On an as needed basis and consistent with 6100 regulations complete Low-Income Home Energy Assistance Program (LIHEAP), rent rebate and Supplemental Nutrition Assistance Program (SNAP)., • Establish and manage individual online portal profiles for application and management of benefits such Social Security Administration (SSA), Medical Assistance (MA), and SNAP, etc., • Electronically submit Medical Assistance documentation to ensure individuals are successfully enrolled and maintained with regard to their MA status., • Maintain a tracking system of anticipated renewal dates for MA benefits., • Update and management Rep Payee Consolidated Client File and provide weekly updates to CLA and Fiscal Management, • Prepare for and participate in routine audits and annual licensing review of financial records., • Participate as needed in individual reviews with outside monitoring agencies., • Meet with Social Security Administrative representatives on behalf of Rep-Payee needs at local SSA branch as needed., • When necessary, participate in in-person redeterminations and other meetings at local County Assistance Office and/or Office of Long-Term Services, • When necessary, participate in court mandated appointments related to MA renewals., • Work with Non-Rep Payee to finalize and address MA Certification and HUD Recertification requirement needs., • In coordination with CLA staff, assure that a signed Room and Board agreement is obtained prior to or upon admission to the CLA residential program. This may require working with HUD Coordinator to ensure compliance with HUD Regulations if the Client will reside in a HUD Subsidy Home., • Responsible for updating Room and Board agreement on an annual or as needed basis to ensure compliance with all requirements. This includes staying on top of income limits/changes that could impact Room and Board., • Assure that residential individuals are not overcharged for Room and Board expenses (in keeping with 6100 regulations) and working with the Fiscal Department to support and reimburse the residents for therapeutic or hospital leave., • Process expenditure requests for use of funds from Representative Payee Accounts. Job Description TITLE: Reunification Support Coach Supervisor DEPARTMENT: DHS/Achieving Reunification Center REPORTS TO: Program Manager PROGRAM OVERVIEW Achieving Reunification Center (ARC), a non-profit entity, has served parents or legal caregivers with children placed in an out of home placement through the Department of Human Services since 2005. The vision of Achieving Reunification Center (ARC) is that all families with children in out-of-home placement have resources and support provided to them to achieve timely reunification. POSITION SUMMARY The Reunification Support Coach Supervisor (RSCS) is responsible for guiding the day to day work of up to five (5) coaches within the RSC Unit. As part of this work, the RSCS is accountable for ensuring that coaches effectively utilize a parent centered coaching model to help parents create goal action plans (GAPS) that directly impact reunification barriers and that the coaches partner with their parents to complete their GAPS. The RSCS conducts regular checks of participant files to monitor GAPs and the delivery of the parent centered coaching model at ARC. Additionally, the RSCS uses data reporting to confirm that parent/caregivers receive high quality support through monthly face-to-face or virtual sessions, as well as follow-ups by telephone and text. The RSCS also uses data reporting to confirm that parents maintain active participation in programming, are connected to needed services and successfully address goals. The RSCS supports coaches to make connections between their work with parents to resolve reunification issues and helping parents to begin the process of strengthening their decision making, persistence and resilience. The RSCS conducts review of reports regarding the progress of parents/caregivers to assigned CUAs and other important parties prior to scheduled Family Court appearances. The role of the RSCS also involves cultivating essential parent centered and motivational interviewing competences and skills through one-on-one supervisory sessions and group meetings. The RSCS also models the ARC organizational values in carrying out the day to day work responsibilities. The RSCS will operate both on-site and remotely. Job Description Position Title: Program Supervisor Reports to: Program Director Department: Administration DOPP Behavior Health HBDS IPS PEP Foster Care TIPS CIC AERC FLSA Status: Exempt Fee-For-Service Non-exempt Location: 5217 Overbrook Ave. POSITION SUMMARY: The AERC Supervisor is an integral member of the AERC team. The AERC Program Supervisor will perform all duties associated with ensuring high quality programming for all on-site and off-site programming. The AERC Program Supervisor will spend most of their time coordinating activities, maintaining a monthly calendar of events, interviewing facilitators, and arranging recreational and cultural enrichment activities. He/she will coordinate all van pick-ups, drop-offs and when needed assist the Van Driver as an additional chaperone during van pick-ups and drop-offs and deputize in the absence of the Program Director. The hours of work are flexible and non-traditional given the many components of the program are based between the hours of 4pm and 8pm. The position is from the 1pm to 9pm, Monday through Friday. The AERC Program Supervisor will also arrange individual and group community service projects for every other Saturday and be present during such activities. He/she will ensure that all community service projects are done in accordance to BARJ standards. He or she will ensure that an accurate and descriptive certificate of completion is granted to each youth who participates in all community service projects and upon successful completion of the program. This position is full time, as the Supervisor will be actively involved in cultivating employment activities, arranging vocational training opportunities, housing opportunities, post educational activities, along with programming at the site. EDUCATION/CREDENTIALS/EXPERIENCE: • Bachelors in one (1) of the following disciplines: Psychology, Social Work, Sociology, Education, Criminal Justice, or other Human Service field is preferred and will substitute for 1 year of experience.
