POSITION SUMMARY: The Hotspotting Opioid Program is an evidence-based program in which a care management model will be led by certified recovery peer advocates and clinical professionals with the objective of providing person-centered services to those at risk of opioid overdose in a safe and collaborative way. The program’s focus is to reduce opioid harm, promote a continuum of recovery, and address critical social determinant of health factors. Under the supervision of the Director of Harm Reduction, the individual will be responsible for providing client outreach, interventions for clients, provide crisis consultations, maintain client case records, linkages to care, referral follow up. Case Managers will document interventions according to all funder and agency requirements, submit statistical data and reports, attend weekly supervision, participate and facilitate case conferences, conduct staff trainings on substance abuse topics, and perform other additional tasks as delegated by supervisor. DUTIES & RESPONSIBILITIES: • Perform outreach to potential clients to engage them into services., • Coordinate the implementation of the multidisciplinary treatment plan, with treatment team., • Document all client activities, progress, and clinical intervention in the case record., • Coordinate treatment services with internal programs and external organizations., • Address and remove barriers to enable client access to all necessary components of health care, including mental health and substance use services by coordinating with the treatment team., • Maintain ongoing face-to-face, telephone, and other collateral contacts to support, encourage, and promote the client's commencement and ongoing compliance with the requirements of treatment and/or services outlined by CHASI and the funder, as well as to assist with mitigating barriers to engagement in services, wellness, and recovery, as well as employability by working closely with the treatment team., • Use required systems to track outreach and engagement activity; conduct Health Related Social Needs Assessments (HRSN) assessments using required platforms and tools and make referrals to treatment and recovery services, • Assist clients in coordinating a manageable treatment., • Help to collect documentation for SSI and housing applications as well as provide advocacy services throughout the application process., • Coordinate logistics for care and treatment needs including appointment reminders and consistent follow-up., • Assess plan and treatment adherence and addresses barriers as they arise., • Connect clients to Harm Reduction services including but not limited to naloxone training and kits, and fentanyl test strips., • Reassess client needs and eligibility for the program., • Develop and discuss aftercare plans with clients, referral resources, and other relevant entities., • Performing crisis intervention and problem resolution for clients in need. CAMBA Housing Ventures·(CHV), the housing development subsidiary of CAMBA, developed these units. 146 of these units are set aside for formerly homeless Individuals and families. 117 units are for Population I Individuals, chronically homeless single adults who suffer from Serious and Persistent Mental Illness or who have a co-occurring Mental Illness and Chemical Addiction (MICA). 15 units are for Population Iii individuals, chronically homeless single adults who have a substance abuse disorder that is the barrier to independent living and who also have a disabling clinical condition. 14 units are for Population V chronically homeless families, In which the head of household suffers from a substance abuse disorder, a disabling medical condition or HIV/AIDS. CAMBA provides on-site case management and supportive services in addition to 24/7 security. Job Description Overview Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Responsibilities The Pharmacy Technician performs all technical tasks in the unit dose, carry and out-patient drug distribution systems as well as other related duties of a satellite pharmacy. The Registered Pharmacist approves all work performed by the Pharmacy Technician. • Unit Dose and Carry Drug Distribution Systems, • Assists in maintaining accurate patient profiles by informing the pharmacist of changes in patient names, I.D. number or housing so modifications can be made in the pharmacy database., • Fills and labels patient cassettes with non-carry and carry system medication bags from computer generated fill lists, and/or original drug orders., • Obtains court medication list from General Office. If patient is on methadone or DOT medications, tells pharmacist of such., • Transports patient cassettes to medication clinic areas and exchanges same when necessary., • Files completed fill lists, updates lists, distribution lists, methadone sheets and other materials according to procedure., • Checks, rotates and restocks all drug stock areas. Fills Pyxis machines as required. Files Pyxis reports with non pyxis stock records and retains them for 1 week. On a monthly basis checks for outdated medications in Pyxis, clinic floor stock, pharmacy stock and other areas where pharmacy stores medications. For non pyxis floor stock, maintains established par levels based on individual clinic or infirmary need. Records issues of stock on the non pyxis Floor Stock Inventory and Issue Record., • Delivers emergency boxes and other requested items to clinic or infirmary areas as needed., • Prepares weekly pharmacy stock order by utilizing par levels and building usage patterns for review by the pharmacist. Enters order into IDWhere and transmits on both a daily and weekly basis to packaging. Reconciles packing slip or invoice against items received. Notifies building pharmacist and packaging manager of any discrepancies. Puts away orders upon receipt, appropriately rotating stocks as required., • Inspects all drug storage areas (clinic, mini-clinic, urgi-center, infirmary, pharmacy, par level rooms, dental, X-Ray, etc.) for outdated medication, IV solutions and related supplies on a monthly basis., • Sorts, counts, prepares outdated drug manifest and packs up outdated medications for return to Central Medical Supply for submission to a reverse distributor., • Prepares and records the prepackaging of extemporaneous unit dose medication., • Records daily statistics on the RX Daily Stats Activity Report form in the computer. Gathers and records other statistical and survey information as required. Emails to appropriate party as requested., • Has a working knowledge of the pharmacy and medical computer systems. Job Description THE ORGANIZATION The Center for Justice Innovation is a nonprofit organization dedicated to advancing community safety and racial justice. Since 1996, we’ve worked alongside communities, courts, and those most directly affected by the justice system to build stronger, healthier, and more equitable neighborhoods. With a team of over 900 staff and an annual budget of $130 million, the Center carries out its mission through three core strategies: • Operating Programs that pilot new ideas and address local challenges;, • Conducting original research to evaluate what works—and what doesn’t; and THE OPPORTUNITY Brooklyn Justice Initiatives (BJI), the largest operating project of the Center, seeks to re-engineer the experience of criminal court in Brooklyn, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Kings County Criminal Court and community-based offices, BJI is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, case management, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant’s life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling. BJI seeks Night Clinical Intake Specialists for the borough’s Supervised Release Program (SRP). Reporting to the Night Clinical Intake Coordinator, Clinical Intake Specialists will leverage social work or mental health training and direct practice experience to facilitate participants’ successful transition into SRP. Clinical Intake Specialists will conduct a comprehensive needs assessment; connect participants with an appropriate case manager, focusing on participant needs and individual case manager skills and strengths; generate mandatory programming recommendations, and refer candidates to services with community-based service providers. Additionally, this position will provide de-escalation and emergency intervention within the courthouse, as needed. Night Clinical Intake Specialists are responsible for effective cross-team communication and collaboration to prioritize comprehensive and aligned service delivery; proactively identifying successes, challenges, and opportunities to strengthen programming; create opportunities for meaningful collaboration, and maximize the use of internal and external resources and expertise.
