Job Description Dietitian Rate: $28- $32/hour (depending on experience) Shift: Full-Time Schedule: Flexible Garden Gate Health Care Facility is looking to add a Dietitian to its team. RESPONSIBILITIES: • Complete and oversee nutritional assessments for newly admitted residents and develop individualized nutrition care plans, • Monitor resident weights, meal intake, supplements, hydration status, laboratory values, and therapeutic diet needs, • Collaborate with the Food Service Director, Diet Technician, nursing staff, therapists, and other disciplines to ensure appropriate nutritional care, • Provide nutrition counseling and education to residents, families, and facility staff, • Review menus, recipes, dietary consistencies, and nourishment programs to ensure nutritional adequacy and regulatory compliance, • Participate in care plan meetings, interdisciplinary meetings, and quality improvement initiatives, • Maintain required documentation, transfer/discharge summaries, and nutrition-related records, • Assist with policy review, dietary education programs, and maintaining the facility diet manual, • Perform additional duties as assigned REQUIREMENTS: • Minimum education requirement: Associate's Degree in Dietetics/Nutrition; Bachelor's Degree in Dietetics/Nutrition preferred, • Minimum experience requirement: Ten full-time equivalent years of education and supervised experience (1,600 hours of experience equals one year of credit for every 30 semester hours of education); completion of Bachelor's program experience requirements preferred, • Valid NYS Dietitian License required, • Strong knowledge of nutritional assessment, care planning, hydration management, therapeutic diets, and food service operations, • Excellent communication, education, and interdisciplinary collaboration skills, • Strong organizational, leadership, and problem-solving abilities, • Ability to maintain regulatory compliance, confidentiality, and professional standards BENEFITS: • Paid Time Off (PTO) for Full-Time and Part-Time, • Weekly or Same-Day Pay Options, • Health, Dental, and Vision Insurance, • Life Insurance, • Employee Referral Bonus Program, • Retirement Saving Options
