This role combines clinical expertise with social work principles to promote client well-being, resilience, and recovery. Key Responsibilities: • Conduct comprehensive mental health assessments, diagnoses (using DSM-5-TR criteria), and develop individualized treatment plans., • Provide evidence-based therapeutic interventions such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), Solution-Focused Therapy, and Trauma-Informed Care., • Facilitate individual, group, and family therapy sessions to address a variety of mental health conditions including depression, anxiety, trauma, substance use disorders, and severe mental illnesses., • Collaborate with psychiatrists, primary care providers, and other members of the interdisciplinary team to ensure integrated care., • Engage in case management activities, linking clients with community resources such as housing, employment services, support groups, and social services., • Maintain accurate, timely documentation of all clinical interactions in accordance with legal, ethical, and agency standards., • Utilize risk assessment tools to evaluate and respond to suicidality, self-harm, violence risk, and other psychiatric emergencies., • Provide crisis intervention services as needed, including safety planning and referrals to higher levels of care., • Educate clients and families about mental health conditions, treatment options, coping strategies, and recovery supports., • Participate in clinical supervision, ongoing professional development, and agency trainings to maintain licensure and enhance clinical skills., • Uphold the highest ethical standards as outlined by the National Association of Social Workers (NASW) Code of Ethics. Qualifications: • Masters degree in Social Work (MSW) from an accredited program., • Active, unrestricted state licensure (e.g., LCSW, LICSW, LMSW in Maryland)., • One or more years of clinical experience in mental health settings preferred (outpatient, inpatient, community mental health, or substance use treatment programs)., • Strong skills in diagnosis, treatment planning, documentation, and clinical intervention., • Knowledge of trauma-informed practices, culturally competent care, and co-occurring disorders., • Proficiency with electronic health records (EHRs)., • Excellent communication, problem-solving, and interpersonal skills. Work Environment: • Office-based, telehealth, or community-based settings., • May involve evening or weekend hours based on client needs and scheduling. Job Description POSITION OVERVIEW The Habilitation Specialist assists individuals receiving services in acquiring and/or maintaining the skills, resources, personal habits, and positive attitudes that will enable them to attain/or enhance healthy personal relationships and make healthy choices and decisions in adult life. SPECIFIC RESPONSIBILITIES • Develop and provide structured services and skill building activities designed to meet the needs of individuals receiving services in accordance with their valued outcomes and assist in the scheduling and planning of same., • Participate in the development and implements the individual habilitation plan (aka rehabilitation plan) for individuals receiving services to reflect their interests, capabilities, and needs, and to enhance their skills and level of independence., • Encourage individuals to express and develop their individual interests, • Provide individuals with opportunities to maintain/ enhance their existing skills, overall functioning and independence, and sense of belonging and community integration through various in-house and community activities., • Assist individuals in achieving their valued outcomes by providing prompts, reminders, guidance and role modeling in areas such as socialization, personal hygiene, exercise, nutrition, behaviors, etc., while involved in skill building activities., • Provide appropriate physical care of assigned individuals to maintain their cleanliness, safety, and comfort. This would include tasks such as direct assistance with personal and home management. This also includes monitoring, correcting, and modeling appropriate behaviors., • Ensure safety and welfare of individuals in their movement and transport., • Develop individual specific programs and provides immediate feedback on performance., • Transport individuals receiving services to external appointments and community resources while also providing community travel training, • Provide self-esteem building opportunities for consumers through recreation, arts, sports, and other community activities, • Assist consumers in developing healthy boundaries and relationships, including within the program, with family, with love interests and other significant others, • Assist in maintaining good physical health by facilitating healthy meal planning, food shopping and meal preparation, • Maintain safe and clean working area and ensures adequate material for goal, • Attend consumer focused and all other required staff meetings as assigned., • Maintain accurate program and statistical records in the electronic health record, including but not limited to daily and monthly progress notes, as well as any other required documentation in accordance with agency and regulatory requirements. • Possess good time management skills, • Ability and desire to work as part of an interdisciplinary team, • Moderate level of written and oral communication skills, • High level of interpersonal skills, • Knowledge of behavioral techniques, behavioral health disorders, and the impact of trauma to assist in the care of individuals receiving services, • Knowledge of computers and ability to utilize word processing programs EDUCATION, WORK EXPERIENCE AND JOB REQUIREMENTS Preferred: Associate or bachelor’s degree in human services plus one-year pertinent experience in providing services to young adults with behavioral health disorders and a valid Maryland driver’s license in good standing. Minimum: High School diploma or GED and valid Maryland State driver’s license in good standing. COMMENTS Must demonstrate knowledge and understanding of mental illness and treatment methodologies. Hours are Monday, Wednesday, Friday11:30am-8:00pm, & Tuesday, Thursday 8:30am- 5:00pm
