Clinic Registered Nurse Job Highlights: • 403(b) Retirement Plan;, • Career scholarships;, • Quality training, continuing career education and leadership programs;, • Medical, Dental and Vision Insurance Pay: $42.50 - $48.00/HR The Clinic RN under the direction and supervision of the Clinic Supervisor, provides skilled nursing care to participants, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family teaching and communicates participant’s condition to primary caregivers, physician and Interdisciplinary Team. Required Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. • Graduate of a school of professional nursing, • Current state Registered Nurses (R.N.) License, • Cardio Pulmonary Resuscitation (CPR) certification required, • A minimum of one year’s experience in working with the frail or elderly population required, • Must have a valid driver’s license, proof of insurance and have means of transportation, • Bachelors of Science Nursing (BSN) preferred. Essentials: • Conducts in-person comprehensive initial nursing assessment with new PACE enrollees. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant., • Conducts an in person nursing re-assessment semi-annually and as needed., • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery., • As needed, provides orientation, initial and annual competencies, of, Licensed Practical Nurse (LPN), Certified Nursing Assistant’s (C.N.A.’s), and Clinic Clerk., • Performs competencies on appropriate day center staff as needed., • Performs general nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards., • Triages participants to determine priority of car Senior Community Care of Michigan - PACE: Senior Community Care of Michigan - PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization.
