Position Summary: The Medical Case Manager is an essential part of the primary care team, focusing on coordinating care, managing chronic conditions, and ensuring that patients receive comprehensive and continuous care. This role involves developing individualized care plans, facilitating communication among healthcare providers, and providing targeted support to improve patient outcomes. Duties and Responsibilities: This job description describes the general nature and level of work performed by employees assigned to this position. It should not be construed as an exhaustive list of all required duties, responsibilities, and skills. • Conduct periodic biopsychosocial assessments to ascertain the level of care required., • Develop and implement individualized care plans in collaboration with healthcare providers., • Make appropriate referrals to community resources to meet patient needs., • Assist clients with enrolling in health insurance., • Connect client to financial assistance programs to cover co-pays, co-insurance, and deductibles., • Facilitate communication between patients, primary care providers, specialists, and other healthcare services., • Monitor patient progress and adjust care plans as needed., • Provide education and support to patients with chronic medical conditions., • Monitor adherence to treatment plans and medication regimens., • Assist patients with navigating the stages of change to improve health outcomes., • Conduct regular follow-up appointments to assess and address patient needs., • Build strong, trusting relationships with patients to encourage active participation in their care., • Conduct patient education sessions on disease management, medication adherence, and healthy lifestyle choices., • Address barriers to care, such as the social determinants of health., • Engage clients in meaningful risk reduction conversations regarding HIV treatment and prevention strategies such as U=U, PrEP, Condoms, and testing., • Encourage healthy lifestyle choices through patient education., • Track and follow up on preventive care measures to ensure compliance., • Complete necessary paperwork and data entry, including entering case notes, within 24 hours of client interaction., • Education Bachelor’s degree in Social Work or related field., • Experience: Minimum of 2 years of experience in case management, preferably in a primary care or chronic disease management setting., • Skills: Strong communication and interpersonal skills, knowledge of chronic disease management, ability to work collaboratively with healthcare providers, and proficiency in patient education., • Experience working with diverse populations and understanding cultural competency., • Experience working with adolescents 16-17 years and their families, especially in healthcare settings, is highly valued. Knowledge, Skills, and Abilities: Possess superior organizational and administrative skills. Excellent interpersonal and communication abilities. Excellent computer skills in a Microsoft Windows environment. Ability to work independently and achieve high standards of productivity, achieving deadlines and with superior product results. Strong calendaring, organizational, and document management skills required. Proactive approach to solving problems and communicating to leadership. Knowledge of office management systems, procedures, and office equipment required. Excellent time management skills and ability to prioritize work. Familiarity with electronic health records (EHR) systems. Ability to conduct health assessments using standardized tools.
