Job Description Home Health Aide (CHHA) Position Overview: A certified Home Health Aide is a person who carries out health care tasks as an extension of a registered professional nurse. A CHHA also provides assistance with personal hygiene, housekeeping and other related supportive tasks for a client with health care needs in his/her home. To administer the home care services that reflect OSH’s mission, vision, and values. Requirements: must be 18 years or older, have active & in good-standing NJ Certified Home Health Aide license Essential Job Functions: • A Home Health Aide is a person who carries out health care tasks as an extension of a registered professional nurse., • A Home Health Aide also provides assistance with Activities of Daily Living & Instrumental Activities of Daily Living according to the Clients Plan of Care., • Participate in implementation of Client plan of care in accordance to OSH policy and procedures., • Ensure quality and safe delivery of home care services that align with OSH, federal and state policies procedures as well as the Occupational Safety and Health Administration and CDC recommendations., • Assists the RN supervisor to make client visits by ensuring presence of self and client at the time planned., • Demonstrates the ability to communicate effectively with the clients, his or her family members, and other members of the healthcare team and staff of the agency., • Reports occurrences to the Nursing Supervisor., • Adheres to universal precautions, aseptic technique and infection control guidelines., • Implements care in a manner that is maximally safe for the client, his/her family, and self., • Seeks, accepts and implements suggestions to improve performance., • Demonstrates respect for the opinion of others., • Consistently assumes and follows through on the responsibility for assignment., • Demonstrates the ability to function effectively under stressful situations., • Maintains confidentiality of client observations and records., • Utilizes time effectively, maintaining a consistent level of productivity., • Completes the continuing education requirements annually (12 hours)., • Complies with standards for attendance, absence notification, and punctuality., • Demonstrates professionalism through appearance, performance and communication., • Assumes responsibility for reading and comprehending all posted notices, communications and policies and procedures related to certified Home Health Aides., • Demonstrates competencies to provide care to patients of all ages., • Respects the rights, privacy and property of others at all times., • Complies with Federal, State and Local Laws., • High School diploma or GED equivalency, preferred., • A certified Home Health Aide: who has completed a CHHA training program approved by the NJ Board of Nursing, has an active CHHA license issued by the NJ Board of Nursing, has completed the Home Care test., • A Nursing Student: who has with a written New Jersey Board of Nursing waiver in lieu of a certificate and has completed the Home care test, • Evidence of a Tuberculosis screening that meets CDC guidelines, • Evidence of competency as per OSH and State requirements., • Must pass OSH written competency exam with a score of 80% or higher., • Excellent communication skills., • Focus on Client Needs: Anticipate, understand, and respond to the Client’s needs, to meet or exceed their expectations within the organizational parameters., • Ability to handle confidential information in compliance with HIPAA., • Ability to handle sensitive information in a calm and professional manner., • Ability to maintain a clean, healthy and safe environment., • Ability to recognize emergencies and knowledge of emergency procedures., • Knowledge of appropriate and safe techniques in all aspects of personal care., • The employee is occasionally exposed to a variety of conditions at Client sites., • Ability to perform tasks involving physical activity, which may include heavy moving, bending, lifting, and extensive self-positioning.
