If you would like to learn more about us, please visit our website at: Position Overview The Community Health Navigation Specialist (“Specialist”) works closely with the Social Care Network (SCN) Lead, Healthy Alliance, and is part of a collaborative, community-based initiative aimed to provide support and connection to services to address unmet Health Related Social Needs (HRSN’s) for individuals and families residing in the Capital District Region of New York State. The Specialist will act as a resource for individuals and their families by being an agent for health-related social needs screening and assessment and a vital linkage to community-based services in order to improve individual and community level health and wellness outcomes. The Specialist will also provide Enhanced Care Management for qualifying individuals. Position Requirements & Specifications • Ability to work: Monday-Friday, 9:00am – 5:00pm w/ flexibility as needed to accommodate client availability., • MSW or Master’s Degree in human service field preferred; BSW or Bachelor’s Degree in human service field AND at least two years’ experience in the human services or mental health field, required., • Local travel is required; A current Clean & Valid New York State Driver’s License (actively licensed at least one year) and reliable vehicle for transporting clients to community resources, meetings, etc. - with proof of 100/300 liability insurance coverage - is required*., • Ability to work with clients with mental health issues, substance abuse and developmental disabilities., • Competent clerical, organizational, and project management skills., • Proficient computer skills: Microsoft Office Suite & Google Business Workspace Suite, and use secure Internet practices., • Ability to effectively navigate electronic health records, adding documentation with high accuracy., • Previous experience working in a human services agency is a plus!, • Strong interpersonal skills, with the ability to build strong working relationships with collaborative partners including school district personnel, community agencies, and social service providers., • Eagerness to learn new theories and practice models to enhance job effectiveness and to achieve program outcomes., • Capacity and openness to work with populations from culturally diverse and economically disadvantaged backgrounds, including but not limited to a broad range of people with various developmental and functional levels., • A professional record of integrity, proven reliability and dependability; Qualities of positivity, flexibility and adaptability., • Capable of exercising ethical, independent judgment and discretion while handling sensitive data with strict confidentiality. Essential Duties & Responsibilities include the following; Additional duties may be assigned by the supervisor. • Accept referrals from Healthy Alliance and/or other local partners and conduct outreach with clients within 2 business days., • Conduct the AHC Health Related Social Needs Screening for all referred clients and eligible members of their households to assess for unmet medical and social needs, and document screening outcomes in the designated Electronic Health Record (EHR)., • Validate Member eligibility for Enhanced HRSN Services and manage closed loop referrals for HRSN Services to ensure all screened clients are connected to appropriate services in the community., • Develop and monitor the “Social Care Plan” for Enhanced Services Population eligible members when applicable., • Navigate clients and their household members to needed medical, mental health and social supports within the community., • Meet with clients in community settings., • Collaborate with internal and external local partners and providers to ensure a coordinated approach to support the member and their family when applicable., • Provide Enhanced Care Management including eligibility assessment, planning, education outreach, facilitation, care coordination, and advocacy services to a caseload of Enhanced HRSN Care Management Services eligible members when applicable., • Provide housing support and sustainability services and resources for eligible clients., • Document all contacts and significant case events in the designated EHR following all agency guidelines.
