Summary: At NeuSpark Person Directed Services, the Registered Nurse must provide comprehensive nursing evaluations for all People we support, determining the most beneficial interventions with expected outcomes. These outcomes will directly affect everyone’s unique capacity and ability to participate as actively as possible in daily life, and to equally enjoy an enhanced quality of life. The sites that will be covered by this position are located in the Bronx and Upper Manhattan: Riverdale, Washington Heights, and Hamilton Heights. Essential Functions and Key Responsibilities: • Work in partnership with the people we support, their circle of support, and the staff in the residence to ensure the rights, health, safety, and desired personal outcomes of those living the residence are always supported., • Assess, identify, and evaluate all the health care needs for the People we support living in the setting., • Provide on-site medical service and direction to staff and People we support within the residence on an ongoing basis by completing physical assessments and making recommendations based on those findings., • Develop, review and update Nursing Care Plans on a quarterly or semi-annual basis depending on Program type. Nursing Care Plans may also be updated as needed., • Maintain all medical records accurately and up-to-date., • Complete Monthly Reports in IRA Programs., • Coordinate with the QIDP, Manager, and other disciplines and identify the medical/Nursing Care Plan components for all People we support presented at treatment planning meetings and prepare for and participate in the Interdisciplinary Team Process with attendance at Quarterly, Semi-Annual, and Annual Review meetings., • Complete records based on the Quarterly, Semi-Annual or Annual Review meetings., • Regularly monitor People we support’ Medication Administration Records (MARs) to ensure they are being administered properly., • Prepare, schedule, and provide needed in-service training for staff in any needed medical-related area., • The RN is responsible for certifying staff upon successful completion of AMAP training and ensure annual recertification updates are completed within the required timeframe and requirements., • Ongoing observation and supervision of AMAP staff in the performance of their AMAP duties., • Report medication errors appropriately and instruct how the error is to be handled to ensure the wellbeing of the person we support. Physical evaluation of people we support subject to such medication errors must be performed., • Follow or establish consistent tracking systems to ensure observation, monitoring reporting, and evaluation of all People we support’ medical concerns, including but not limited to: sleep charts, BM charts, weights, vital signs, menstrual records, seizure charts and any other record deemed medically necessary., • Monitor for adverse effects of medications, including allergies, interactions, and contraindications., • Prepare People we support for specialized treatments, tests, or procedures in accordance with Agency policies and informed consent. Ensure all medical appointments are scheduled and follow-up appointments are made as needed. Ensure Residence Manager has needed information regarding these appointments., • Review Clinic summaries and, in collaboration with the QIDP and Manager, ensure all Physician orders and recommendations are followed., • Review and initial all medically related documentation., • Engage in direct communication with physicians, either telephonically or in person, to review and clarify clinical recommendations. Serve as the designated liaison between the residence and the medical providers/ facilities, ensuring the timely resolution of questions, concerns, or issues that may arise., • Maintaining accurate, concise, and timely medical documentation on each person we support’s heath status., • Follow OPWDD regulations and standards regarding the implementation of all nursing/health-related procedures, protocols, and services., • Ensure availability for 24/7 on-call telephone accessibility to answer questions and respond to medical emergencies., • Mandatory attendance at required in service training, along with ability to absorb and demonstrate compliance with all Regulations and Agency policy and procedure requirements is required for this position., • All Employees will be required to learn and master the use of any and all software systems and technology utilized by the Agency on a Program by Program basis., • Covering RN vacancies as needed., • Current NYS RN Licensure and Registration in New York State, • Clinical experience in a hospital, adult care medical facility, or OPWDD is a plus, • Experience with Adults with Intellectual and Developmental Disabilities, • Must be flexible as necessary with work schedule NeuSpark Person Directed Services is committed to fostering a welcoming, inclusive, and respectful environment for our employees, contractors, and the diverse people we support and families we serve.
