Patient Scheduling Specialist (Temporary)

Job Description Position Description The Patient Scheduling Specialist (Temporary) plays a critical role in ensuring an efficient, patient-centered experience at Project HOPE. This role is responsible for managing patient scheduling across the continuum of care, from initial appointments to follow-ups, while coordinating prior authorizations, durable medical equipment (DME) processes, and check-out procedures. The ideal candidate is detail-oriented, proactive in-patient outreach, and excels in providing exceptional service in a fast-paced, mission-driven clinical environment. Primary Tasks & Duties: Scheduling & Patient Communication • Schedule all patient appointments, including follow-ups, routine care, nurse visits, and same-day needs., • Proactively contact patients in advance of appointments to confirm attendance or offer rescheduling., • Reach out to patients who miss appointments and reschedule promptly., • Fill same-day appointment slots with walk-ins or hospital follow-ups as needed., • Schedule telehealth visits when appropriate, in coordination with the receptionist and clinical team., • Ensure every patient seen leaves with a future appointment scheduled., • Check-Out and Documentation., • Manage patient check-out, verifying that all required documentation is complete., • File completed forms and ensure incomplete forms are placed in the appropriate provider folders for review., • Track all patient appointments and support accurate documentation in the Electronic Medical Record (EMR) system., • Prior Authorizations & DME Coordination, • Complete and submit prior authorization requests for medications, procedures, and other services as required., • Coordinate the DME process from start to finish—including paperwork completion, provider signature collection, communication with vendors, and follow-up with patients to confirm receipt., • Maintain records and logs to ensure timely delivery and resolution of DME services., • Clerical and Operational Support., • Assist with administrative and clerical tasks in support of the clinical team, including faxing, scanning, filing, and distributing communications., • Generate patient-related letters using approved templates and secure necessary provider signatures., • Participate in daily morning huddles and support general clinic flow., • Track and manage patient referrals from initiation to completion, ensuring timely follow-up and documentation., • Address barriers and support access to ensure patients complete referred care. Other Essential Functions: Clerical and Administrative Support • Perform clerical duties such as faxing, scanning, distributing provider messages, and organizing patient records., • Generate patient letters using pre-approved templates and ensure provider signatures are obtained., • Sort and distribute clinical mail., • Technology and EMR Use, • Accurately input and retrieve information using the Electronic Medical Record (EMR) and scheduling systems., • Maintain documentation logs and update patient status and appointment history in real time. Team-Based Workflow Participation • Actively participate in daily huddles and team-based care planning discussions., • Coordinate tasks and follow-ups with clinical staff to support integrated care delivery., • Customer Service and Confidentiality, • Provide courteous, responsive, and culturally sensitive service to all patients and families., • Maintain strict patient confidentiality in compliance with HIPAA and organizational policy. Minimum Qualifications: • High School Diploma or equivalent required., • At least one year of experience in a healthcare or patient-facing administrative role., • Strong customer service and communication skills., • Proficiency with EMR systems and scheduling software., • Ability to multitask and manage competing priorities with accuracy and efficiency., • Demonstrated reliability, punctuality, and the ability to work both independently and as part of a team. Preferred Qualifications: • Associate degree or additional certifications preferred., • Experience working with underserved communities., • Bilingual in Spanish/English strongly preferred., • Familiarity with DME and prior authorization processes., • Experience working in Federally Qualified Health Centers (FQHCs) or community health settings. Physical/Environment: • Occasional lifting of 40–50 lbs. and pushing of 5–20 lbs., • Sitting, standing, and walking approximately 80% of the time., • A medium to high level of manual dexterity required., • Bending and reaching approximately 20% of the time., • Normal accessibility and mobility throughout the region required., • Occasional overtime/extended work hours may be required based on operational needs. Work Environment Indicators: ☑ Normal community health working environment. Requires good verbal and written communication skills. Must be able to speak and read the English language. ☑ Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of fifty (50) pounds without assistance. ☑ Work is primarily facility-based in an outpatient behavioral health setting and/or correctional facility. ☐ Some work may occur in a community setting and may include contacts in a high crime area and in extremes of temperature. Compensation & Benefits: Hourly Rate: $24.00 per hour; $49,920.00, annualized, commensurate with experience and qualifications.
