Work Schedule: Full-time/40 Hours Position Type: Hybrid Branch Location: Liverpool, NY The RN Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel. This role is a focused on both home health clinical quality assurance and home health clinical operations initiatives. * Develops, plans, implements, analyzes, and organizes clinical operations for a specific location managed. * Conducts/delegates the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies. * Manages the assignment of caregivers. * Responsible for and oversees the delivery of care to all patients served by the location. Receives case referrals. Reviews available patient information related to the case, including disciplines required, to determine home health or hospice needs. Accountable to ensure patients meet admission criteria and make the decision to admit patients to service. Assigns appropriate clinicians to a case, as needed. * Instructs and guides clinicians to promote more effective performance and delivery of quality home care services and is available at all times during operating hours to assist clinicians as appropriate. * Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing patient Plan of Care (POC). * Monitors cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers. Ensures final audits/billing are completed timely and in compliance with Medicare regulations. * Coordinates communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning. * Works in conjunction with the Branch Director and Company Finance Department to establish location’s revenue and budget goals. * Participates in sales and marketing initiatives. * Supervises all clinical employees assigned to a specific location. Responsible for the overall direction, coordination, and evaluation of the location. Carries out supervisory responsibilities in accordance with Company policies and procedures. * Handles necessary employee corrective action and discipline issues fairly and objectively, in consultation with the Human Resources Department and the Executive Director/Director of Operations. * Participates in the interviewing, hiring, training, and development of direct care clinicians. Evaluates their performance relative to job goals and requirements. Coaches staff and recommends in-service education programs, when needed. Ensures adherence to internal policies and standards. * Assesses staff education needs based on own the review of clinical documentation in addition to feedback and recommendations by Utilization Review staff. Upon completion of the assessment, creates and conducts regular staff education as needed. * Analyzes situations, identifies problems, identifies and evaluates alternative courses of action through the utilization of Performance Improvement principles. * Responsible for review of the appropriate number of Case Managers and clinical staff documentation to include starts-of-care, resumption-of-cares, and re-certifications, for appropriateness of care, delivery, and documentation requirements. * Responsible for the QA/PI activities. Works with Utilization Review staff relative to data tracking for performance review and outcomes of care analysis to determine efficiency, the efficacy of case management system as well as any other systems and process. Competently performs patient care assignments and staff management activities. * Provides direct patient care on an infrequent basis and only in times of emergency. * Acts as Branch Director in his/her absence. * Interprets Company standards and Company policies and procedures to ensure compliance with external regulatory authorities and ensures that caregiver clinical documentation meets internal standards. * Participates in performance improvement activities, maintains ongoing clinical knowledge through internal and external training programs. Provides interpretation of knowledge and direction to staff. * Maintains relationships with referral/community sources. Participates in professional organizations and conducts care-related programs. * Performs other related duties as assigned or requested. Required Experience/Skills: * Graduate of an accredited School of Nursing. * BSN required (NY state requirement) * Current state license as a Registered Nurse. * Proof of current CPR. * Valid driver’s license, auto insurance and reliable transportation. * Two years as a Registered Nurse with at least one-year of management experience in a home health environment. * Certified Home health Agency experience is required (NY state requirement) or four years' CHHA experience and six credit hours toward BSN. * Management and people leadership experience, preferred. * OASIS experience, required. OASIS certification (HCS-O, COQS, and/or COS-C), preferred. * Homecare Homebase (HCHB) experience, preferred. * CMS PDGM billing knowledge and/or experience, preferred. Ensures that the standard of dialysis provided is at least equivalent to that provided to facility patients., • Oversees the identification and evaluation of candidates for the Home Therapies Program including assessment of patients' home environment if needed., • Accountable for timely completion of patient care plans by organizing meetings of the health care team to discuss patient care plans and resolve patient problems., • Maintains and communicates efficient and timely patient schedules to ensure maximization of the facility station efficiency., • Ensures completion of Initial and Annual Nursing assessment, and ongoing evaluation and documentation of patient care needs according to FMC Policies and Procedures is completed., • Ensures patients' response to home dialysis treatment therapy is documented according to company policies and procedures and in compliance with pertinent regulatory requirements., • Ensure appropriate intervention for changes in patient adequacy status and troubleshooting peritoneal and access flow issues are completed per physician orders., • Identifies and communicates patient related issues to the physician., • Develops action plans for missed treatments in collaboration with the Medical Director., • Administers medications as prescribed including algorithm (as appropriate), and document appropriate medical justification if indicated. Responsible for the appropriate recording of controlled substances as required by law., • Ensures reports of alert/panic and abnormal lab results are communicated to appropriate physician., • Implements staffing and medical supply models to ensure provision of quality patient care., • Ensures all educational needs of patient and family are met regarding End Stage Renal Disease (ESRD)., • Ensures education of the patient regarding quality measures, transplant options, modality awareness, and access care, including vascular catheter reduction and adherence to treatment regime., • Oversees coordination transplant education and assists in transplant workup process., • Ensures all patients are educated regarding laboratory values and the relationship to adequate dialysis therapy, compliance with treatment schedules medications, and fluid as appropriate., • Oversees the safe, effective and timely training of all home patients and their families/significant others on the safe and effective operation and maintenance (as appropriate), of all home therapies equipment, • Acts as a resource for the patient to address concerns and questions., • Develops a mechanism or process for acquiring and maintaining knowledge of the specific situation of each patient, including hospitalizations, and any significant change in patient care status., • In the absence of a Kidney Care Advocate, the HT PM will manage the delivery of the Treatment Options Program., • Manages and ensures all physician orders are accurately entered into the Medical Information System and that company policies and government regulations related to the collection, packaging, and delivery of lab specimens and reporting of results are followed., • Manages assigned program Home Therapies licensed personnel, direct patient care staff, indirect patient care personnel, and when necessary, technical staff., • Provides employee education and guidance, and performance related feedback through ongoing informal advice and formally through the annual performance evaluation., • Manages the Home Therapies staffing through the appropriate hiring, firing and disciplinary actions., • Ensures completion of new hire orientation and training including mandatory in-services and ICD-9 code training when applicable., • Collaborates with staff to establish personal goals and provides opportunities for professional growth and training to achieve these goals to ensure clinical competence., • Conducts regular staff meetings to ensure appropriate communication of area, regional and corporate initiatives and policies and procedures, to educate and monitor staff regarding appropriate patient care techniques and other company and regulatory policies and procedures., • Responsible for all patient care employees receiving appropriate training according to policy, including training to ensure ongoing compliance with all risk management initiatives. Ensure documentation completed for all annual in- service training., • Collaborates with all appropriate departments to promote home therapy growth, • Provides written documentation of all disciplinary conferences in accordance with the established personnel policies, and confer with the Director of Operations regarding the nature of the disciplinary decisions., • Participates in Corporate and Business Unit specific employee recognition and satisfaction programs., • Creates, maintains and communicates efficient and timely employee schedules according to the needs of the facility., • Ensure all staff is trained in the principles and concepts and practice Customer Service Model., • Creates and implements a (CQI) Process Improvement Team that involves staff in problem solving., • Ensures competency of HT personnel in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone., • Directs staff and patients in the preventive maintenance of home therapy equipment, including water systems and analyses as appropriate. Job Description The Nurse Residency Program provides a robust 12 week orientation. This includes classroom time with a Clinical Educator, and under the supervision of an experienced Preceptor, guided hands-on experience in the home visiting nurse realm. Role and Responsibilities The RN Case Manager will administer skilled nursing care to patients requiring professional nursing service. Teach and supervise the family and other members of the nursing team. Perform these services in accordance with the physician’s orders. Essential Functions • Provide and coordinate quality patient care, maintain a full caseload, maintain productivity goals with consideration of case mix, and accurate, timely documentation., • Responsible to manage each patient’s care as efficiently and cost-effectively as possible, to achieve positive patient outcomes regardless of payer., • Responsible for the overall plan of care and the outcome of that care, including care delivered by other nursing staff (LPN, HHA) who must be directed and supervised by the RN Case Manager., • Conduct admission and manage care of patient to the agency by:, • Obtaining a medical history, particularly as it relates to the present condition, from the patient and/or family member(s)., • Conducting a physical examination of the patient including vital signs, physical assessment, mental status, appetite and type of diet, assessment of ADL, and activity of daily living., • Evaluating the patient, family member(s), and home situation to determine what health teaching will be required., • Evaluating the patient’s environment to determine what assistance will be available from family members in caring for the patient., • Evaluating the patient’s condition and home situation to determine if the services of a home health aide will be required and the frequency of this service., • Interpreting nursing and other services of the agency to patients and families as a part of planning for care., • Meeting regulatory requirements and having a comprehensive understanding of various insurances., • Practicing accepted nursing standards with regards to infection control., • Develop and implement the nursing care plan., • Provide skilled nursing care as outlined in the nursing care plan to include the following:, • Skilled nursing services, treatments and preventive procedures ordered by the physician., • Initiating preventive and rehabilitative nursing procedures as appropriate for the patient’s care and safety., • Observing signs and symptoms and reporting to the physician reactions to treatments, including drugs and changes in the patient’s physical or emotional condition., • Teaching, supervising, and counseling the patient and caregivers regarding the nursing care needs and other related problems of the patient at home., • Oversee the care given by Home Health Aides to assigned patients by:, • Supervising and evaluating the care given by the Home Health Aide., • Applying documentation according to standards of nursing practice., • Utilizing computer skills for accurate documentation., • Document those services rendered to the patient, note changes in the patient’s condition and/or family and home situation, make revisions in the nursing care plan as needed, record supervisory visits conducted with the Home Health Aide, evaluate patient care and progress and close charts of discharged patients.
