Job DescriptionWe are a well-established, family-owned home health agency serving San Antonio and surrounding areas for over 30 years. Our organization is built on stability, compliance, and clinical excellence. We are seeking a highly organized, motivated RN or experienced LVN to join our clinical operations team as an Office-Based Case Manager with occasional field responsibilities. This is not a remote position. This is a hands-on, accountability-driven role for a nurse who thrives in structure and takes pride in clean documentation, strong communication, and operational excellence. About the Role This position is primarily office-based and focused on skilled home health case management and clinical coordination. The ideal candidate will support daily operations, ensure timely physician communication, manage documentation flow, and assist in maintaining compliance standards. Occasional field visits may be required for supervisory or urgent patient needs. Responsibilities Clinical Case Management & Coordination • Review and process physician orders, plans of care, and verbal orders, • Track and follow up on unsigned physician orders until completed, • Communicate with physician offices regarding updates, changes, and clarifications, • Coordinate with field clinicians regarding patient status changes, • Ensure timely and accurate documentation in EMR, • Monitor hospitalizations and track discharge follow-up, • Participate in case conferences and interdisciplinary communication, • Support compliance with Medicare and state regulations Documentation & Compliance • Maintain audit-ready documentation standards, • Ensure timely completion and follow-up of orders and care plans, • Identify and escalate documentation gaps, • Assist in maintaining strong clinical oversight processes Field Support (Occasional) • Perform supervisory visits as required, • Conduct urgent or problem-focused visits when needed, • Provide clinical support and assessment when escalation is necessary Qualifications Required: • Active Texas RN or LVN license in good standing, • Minimum 1 year of home health experience (Medicare experience preferred), • Strong understanding of physician order management, • Experience communicating directly with physicians and clinical staff, • Comfortable with EMR systems, • Strong organizational and time management skills, • Willingness to occasionally perform field visits Preferred: • Skilled home health case management experience, • OASIS familiarity (RN applicants), • Experience in order tracking and compliance monitoring, • Bilingual (English/Spanish) a plus Who We Are Looking For We are looking for a nurse who: • Takes ownership of their workload, • Does not require micromanagement, • Is proactive in following up on outstanding orders, • Understands the importance of documentation, • Is comfortable picking up the phone and resolving issues, • Can work independently while supporting the clinical team This role is ideal for a nurse who wants stability, leadership exposure, and the opportunity to grow within a respected, long-standing agency.
