The Self- Directed Program provides as needed care to individuals as assigned, to meet their daily needs and enable them to function at the highest possible level. Effectively supports ongoing and collaborative communication amongst all program staff and is responsible for ensuring a trauma informed care environment. Essential Duties and Responsibilities • Assists individual with specific facilitative outcomes as described in each individualized Self-Directed Plan. These duties may include personal care, community integration, money management, advocacy, employment needs and respite., • Continually seeks opportunities to meet individual’s psychosocial needs through both care planned and spontaneous actions., • Provides reliable, safe, high-quality services as directed, in a thorough and caring manner., • Implements a person-centered approach to services., • Follows universal precautions, proper infection control, sanitation, and safety standards of practice in all work and activities., • Completes paper time sheet/invoice and web- based time keeping in a timely manner in accordance with payroll processing., • Communicates with supervisor and Circle of Support about individual’s habilitation plans that may need revisions or updates., • Communicates effectively with individuals with intellectual/developmental disabilities, the Circle of Support, Broker, Fiscal Intermediary Coordinator, service coordinator, supervisor, interagency professionals, community professionals and families., • Consistently works with all team members, led by individual and family preference to meet the needs and desires of the individual served., • Respects the individual’s dignity, privacy, property, religion, and culture., • Documents and maintains accurate records of contacts with individuals., • Provides appropriate interventions to all crisis situations. This position requires exemplary customer service and attention to detail. KEY RESPONSIBILITIES: Other duties may be assigned • Before starting trip, walks around truck to observe any visual defects and checks operating systems such as brakes, horn, windshield wipers, lights, first-aid kits, emergency flashers; completes all required inspection sheets, turning them into appropriate personnel., • Loads cambro (insulated food carrier) units and meal components in full, • Picks up and delivers food items and cambros by following prescribed service route and safety rules., • At end of route, checks truck interior, removing any waste items or trash., • Completes all driver logs and paperwork associated with pick-ups and deliveries in a timely manner.
