Physical Therapist JOB SUMMARY Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include but are not limited to assessment; planning, providing treatment; and teaching participant, caregiver or other appropriate representatives/family to maintain participant support in the community. ESSENTIAL DUTIES AND RESPONSIBILITIES: • Provides high quality care and serves as a member of the PACE interdisciplinary team (IDT)., • Performs in-person assessments for participants at initial enrollment and as-needed ongoing as determined by the IDT., • Assesses the need for adaptive durable medical equipment, specialized shoes and prosthesis. Ability to assess participant’s home for safety and mobility. Recommend home modifications and equipment., • Plans for physical therapy treatment programs according to recognized standards and policies., • Administers physical therapy treatment programs consistent in achieving identified goals and according to recognized standards and policies. Confer with interdisciplinary team on participant care issues., • Documents evaluation, assessment, plan and treatment rendered accurately and in timely according to policy., • Measures effectiveness of treatment, document and revise plan accordingly., • Educates and instructs participants/family in procedures, home programs and document training and understanding by participant/family., • Trains PACE staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing activities of daily living (ADL’s)., • Provides care and services in the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care., • Assists participant to function at most independent level possible., • Develops and maintains positive relationships and communication with co-workers, participants and their families/significant others, and members of the community., • Assists the interdisciplinary team members in understanding the significant rehabilitation needs of the participant and families \, • Collaborates with the interdisciplinary team to develop a comprehensive care plan for each participant., • Maintains regular attendance at and participates in daily Interdisciplinary Team meetings, communicates participant changes and collaborates with team members in care planning decisions and coordination for 24-hour care delivery., • Performs the duties of other IDT members based on professional licensing, competencies, and experience as needed., • Actively participates in quality improvement projects and meetings., • Fulfills the role of other IDT member(s) as needed according to licensure and competency requirements., • Attends and actively participates in a variety of organizational meetings related to participant care or daily operations, in-services and community agency meetings., • Completes all documentation of clinical service in participants’ medical records, including initial assessments; reassessments; change of status; and other significant events according to PACE documentation requirements., • Follows all Policies and Procedures and OSHA safety guidelines., • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families., • Practices standard precautions., • Maintains a safe working environment, following safety policies and procedures., • Participate in continuing education classes and any required staff and training meetings., • Maintains professional affiliations, required certifications and continuing education requirements, • Advanced degree in Physical Therapy from an accredited school of Physical Therapy, • Current license to practice Physical Therapy in the state, • Current driver’s license and proof of auto insurance, • Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable., • Match begins after one year of employment Full-Time- Days Full-Time

Job Description Summary The Acute Dialysis Unit provides inpatient dialysis services at 2 locations: MUSC Main Hospital has a 10-bed unit located on the fifth floor of the Main Hospital Building. Ashley River Tower and Shawn Jenkins Children’s Hospital is supported with a 4-bed unit located on the 6th floor of ART. Service is provided 24/7. The two Dialysis Units are fully staff Monday through Saturday 06:30-19:00. After hours/Sundays are covered by limited in-house and on call coverage. Due to clinical needs, we may run patients in their inpatient room as a bedside procedure. Patients commonly treated by the dialysis unit include acute and chronic renal failure patients admitted as inpatient under various medical services for various acute conditions. We also support Renal Transplant patients working through their transplant process. These patient populations do include both Adults and Pediatrics. Primary services include Hemodialysis, but we also support transfusions, antibiotic therapy, and peritoneal dialysis support. The ART operating room offers adult outpatient and inpatient surgeries through various surgical service lines including general, plastics, oncology, cardiac, thoracic, heart and lung transplants, vascular, colorectal, bariatric, and robotics. Operations are Monday through Friday, 7:00am to 7:00pm. General, Vascular, and Cardiothoracic call teams are available to cover emergency cases prior to the posted schedule times and on the weekends. There is 24-hour charge nurse coverage for after hours and weekends. The clinical programs that service ART are recognized for excellence in cardiac surgery, thoracic surgery, vascular surgery, transplant surgery, as well as general and GI surgery and is nationally recognized for management of chronic pancreatitis, and bariatric surgery.
