Job Summary The Director of Community Partnerships and Integration provide strategic leadership for the network’s community integration efforts, serving as the primary facilitator and steward of relationships between Community Health Centers (CHCs), community-based organizations (CBOs), and regional and national partners. This role oversees partner engagement with Carina’s technical infrastructure, identifies and formalizes key community partnerships, and ensures effective integration of community-based services into clinical workflows. By strengthening community-clinical linkages, this position directly supports improved health outcomes and access to care, reduces avoidable utilization, and enhances network performance across quality, cost, and utilization metrics. The Director plays a critical role in solidifying the network’s position as a trusted community connector and community technology leader while aligning community partnerships with value-based care objectives. The role collaborates closely with analytic and quality staff to identify preferred specialist partners for outreach. This position is also responsible for hiring and managing Community Health Workers (CHWs). What You’ll Do Community & Organizational Partnerships • Identify, cultivate, and manage strategic partnerships with national organizations (e.g., ADA, AMA, AHA, ACS) and local community-based organizations, including nonprofits, schools, local governments and social service agencies., • Develop partnership frameworks, MOUs, and engagement strategies aligned with organizational mission, values, and network priorities., • Strengthen relationships between CHCs and community partners to support referrals, care coordination, social needs resolution, chronic disease management, prevention, and health equity goals., • Serve as the network’s lead for assessing and supporting Carina’s community integration strategy and partnerships., • Collaborate with the Data Engineering department to ensure effective bidirectional referral workflows between CHCs and community-based partners, including closed-loop referrals and outcome tracking., • Works with internal staff to:, • Analyze referral patterns, cost, quality, and utilization data, • Identify and recommend preferred specialists and community partners, • Support development of network-aligned referral pathways, • Hire and provide strategic and operational oversight of Community Health Workers (CHWs) focused on in-home visits and gap closure., • Ensure CHW activities align with quality measures, utilization reduction goals, and care team workflows., • Align community integration activities with Carina’s value-based contracts, quality measures, and population health priorities., • Monitor and report on the impact of community partnerships on outcomes, utilization, and patient experience., • Identify opportunities to scale high-impact partnerships and sunset low-value activities., • Establish Carina’s statewide network of community partnerships.Communication & Relationship Management, • Serve as the primary point of contact for community partners, • Facilitate regular communication, meetings, and progress updates, • Collaborate closely with Quality, Analytics, Care Management, and Clinical Leadership teams., • Support CHCs in operationalizing community partnerships within clinic workflows., • Serve as a connector between external partners and network stakeholders. What We’re Looking For • Bachelor’s degree required; Master’s degree in Public Health, Health Administration, Social Work, or related field preferred., • Minimum of 7–10 years of experience in community partnerships, population health, care integration, or value-based care environments., • Demonstrated experience working with CHCs, CINs, ACOs, or similar provider networks., • Strong understanding of social drivers of health and community-based service delivery, • Experience managing teams, including CHWs or community-facing staff., • Ability to translate data and analytics into operational and partnership strategies., • Experience with Federally Qualified Health Centers and safety-net support services, • Experience leading or participating in Community Information Exchange initiatives., • Familiarity with value-based payment models, quality measurement (e.g., HEDIS, Stars), and utilization management., • Experience developing preferred provider or specialist networks., • Strong external-facing presence with the ability to represent the network across diverse stakeholder groups.
