Job Description Role and Responsibilities Under the direction of the department manager, the Clinical Educator is responsible for assessing, planning, implementing, and evaluating an organized program across the footprint, for staff orientation and ongoing continuing education, directed toward the achievement of professional excellence and regulatory compliance. Works collaboratively with the department manager to provide standardized, effective, and efficient education across the footprint. Serves as the primary contact person for professional staff orientation. Essential Functions • Organize and oversee orientation for new field- and office-based staff., • Support the development of continuing education of clinical and paraprofessional staff., • Continually evaluate program effectiveness and relevancy., • Participate in the development and teaching of classes/in-services hosted in person or through electronic learning management system., • Organize and facilitate clinical competency testing., • Assist in coordination of HCR Preceptor program., • Perform co-visits with field staff as needed., • Track staff requirements for in-service training., • Provide or facilitate training in a variety of different formats to meet the needs of field staff., • Work closely with LHCSA management and other agency personnel to coordinate home health aide education across services., • Assist with the evaluation and organization of similar programs at branch offices as needed., • Develop training materials for field and office staff using various training methods, techniques, and formats., • Organize and maintain up-to-date educational materials for staff use and reference., • Direct remedial training as identified by managerial staff., • Standardization of patient educational materials., • Other duties as assigned. Education Requirements • Graduate of accredited nursing (RN) program., • Bachelor’s degree in nursing preferred. Qualifications and Requirements • Current, valid RN license issued by New York State Department of Education required., • Minimum of one year of homecare experience required., • Proficient in Microsoft Office., • Excellent customer service skills., • Knowledge of State and Federal Regulations and Medicare/Medicaid Guidelines for home care., • Knowledge in theories of teaching and principles of adult education (preferred)., • Ability and desire to serve as a role model., • High level of OASIS knowledge and competence; Certificate for OASIS Specialist- Clinical (COS-C) preferable but not required., • Demonstrate effective written and verbal communication and strong organizational skills., • Demonstrate ability to travel to HCR branch locations as needed. Work Environment The Clinical Educator is primarily in an office setting and may be exposed to outdoor conditions. The working conditions are classified as light work: • Light work - Exerting up to 20 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Pay Rate: $18.00-$23.00 per hour Per Diem General Scope: The Medical Assistant is responsible for a variety of patient care, technical, and clerical related functions. The Medical Assistant is responsible for the clinic patient flow by assisting members of the patient care team and providing a broad range of health care services. The Medical Assistant may also be involved in the training and guidance of clinical staff. As a member of the patient care team, the Medical Assistant is also aware of the psychosocial needs of patients and families. Education / Qualification: High School Diploma or equivalent Completion of an accredited Medical Assistant program or Licensure as an LPN. In lieu of the above requirements, two (2) years of recent Medical Assistant experience. Essential Functions: • Perform patient check in, including welcome patients to practice, update demographics, insurance information, make sure all check in forms are signed in records. Check out patients as well., • Administer medications and immunizations orally, intramuscularly, subcutaneously, intradermally, and by inhalation as ordered by provider. Perform point of care testing order and phlebotomy, • Review medical records for up-to-date problem list including patient reported medication and allergies and diagnoses that are documented by physicians or providers., • Participate in patient education by providing standardized, structured written/verbal information to patients regarding procedures, preparation for diagnostic tests, and chronic disease conditions as instructed by physician or manager., • Provide emergency care to patients when necessary using basic life support, • Assist provider with procedures including those requiring aseptic and sterile technique., • As applicable to department operations, maintains competency and performs standardized phlebotomy procedures, basic specimen processing, and limited waived testing as delegated., • Document complete and accurate patient observations, care, and outcomes of interventions in the electronic medical record.., • Provide clerical support to facilitate optimal patient care and area operation., • Demonstrate proficiency in use of the computer for all aspects of patient care and documentation., • Facilitate the coordination of patient services within the clinic and other departments, e.g. radiology, clinical lab, specialist referrals etc., • Answering phones and facilitating messages to right provider., • All other job duties as assigned. Experience: Minimum of two (2) years of recent Medical Assistant experience.
