The Licensed Clinical Social Worker (LCSW) will provide a wide range of social work services to patients across various care settings within the Mohawk Valley Health System. The LCSW will be responsible for conducting assessments, developing treatment plans, providing therapeutic interventions, and collaborating with a multidisciplinary team to ensure holistic and effective patient care. May diagnose and provide counseling for mental health conditions independently. The LCSW and may work in hospital inpatient or outpatient settings. Patient Assessment & Evaluation: • Conduct comprehensive biopsychosocial assessments to identify patient needs, strengths, and challenges., • Evaluate patients’ mental health, social, and environmental factors that may impact their overall well-being., • Develop and implement individualized treatment plans in collaboration with patients, families, and the healthcare team., • Provide individual, group, and family therapy sessions to address psychological, emotional, and social issues., • Support patients in managing chronic conditions, adjusting to life changes, and addressing psychosocial stressors., • Coordinate and manage care transitions between hospital, skilled nursing, and outpatient settings to ensure continuity of care., • Assist patients in navigating and accessing community resources and support services., • Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure integrated and comprehensive patient care., • Maintain accurate and timely documentation of patient assessments, treatment plans, progress notes, and other required records., • Provide education to patients and families about coping strategies, mental health conditions, and available resources., • Provide emotional support and guidance to help patients and families navigate complex healthcare situations., • Offer training and support to staff members on social work practices and psychosocial issues., • Engage in ongoing professional development and training to stay current with best practices in social work and healthcare., • Contribute to quality improvement initiatives and program development within the health system., • Meet CEU requirements to maintain licensure., • Perform related duties as assigned. REQUIRED: • Master’s Degree in Social Work (MSW) from an accredited institution., • Minimum of 3 years of clinical experience, with a background in hospital inpatient or outpatient settings., • Strong understanding of social work practices, ethical standards, and relevant healthcare regulations., • Excellent communication, interpersonal, and organizational skills., • Ability to work collaboratively within a multidisciplinary team and manage a diverse caseload., • Proficiency with electronic health records and documentation practices., • Ability to handle stressful situations with professionalism and empathy., • Experience with EPIC. REQUIRED: • Current and valid New York State licensure as a Clinical Social Worker (LCSW).
