Are you passionate about delivering structured diabetes education to patients living with type 2 diabetes? Then welcome to the DESMOND Education Programme. We have an exciting opportunity for a motivated, forward‑thinking and well‑organised Band 3 DESMOND educator to join our friendly and supportive team. DESMOND (Diabetes Education and Self‑Management for Ongoing and Newly Diagnosed) is a NICE recognised national programme. It is an established education programme designed to give people with Type 2 diabetes a good start in managing their condition. Courses are delivered by two accredited DESMOND Educators who work together to facilitate group education. DESMOND educators must follow the prescribed training programme set by the DESMOND National Programme. As an educator you will be expected to attend DESMOND training to develop and maintain accreditation, to work with another DESMOND support worker to deliver approved structured patient education programmes for self‑management of diabetes, to cover short‑term planned and unplanned leave in line with DESMOND guidance and the Trust lone‑working policy, and to maintain your qualification and competency. We value our staff, believing that ‘happy staff makes for happy patients’ and have a recognised track record in staff engagement and living our values. • People at the Heart, • Listen and Involve, • Kind and Respectful, • ONE Team, • Deliver the DESMOND patient‑structured education programme to groups of people with Type 2 diabetes from the Wigan area in a consistent, coordinated manner., • Assist people with Type 2 diabetes to reflect on their current thoughts and feelings about their diabetes and its impact on their lives, to actively consider the health risks they face and to work towards setting realistic short‑term behavioural goals for themselves., • Respond to demanding patient situations as and when they arise., • Be committed to the DESMOND philosophy and principles., • Assist with the planning, co‑ordination and delivery of Diabetes Education sessions., • Ensure the educational space is prepared for the session and free of any potential hazards or problems, and report any problem to senior staff., • Work with the Diabetes team's administration staff to ensure courses are booked and relevant administration tasks are completed in relation to the provision of DESMOND courses., • Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team's performance., • Assess own performance and take accountability for own actions, either directly or under supervision after each DESMOND course., • Effectively manage own time, workload and resources and contribute to the overall team‑working of the service., • Alert senior team members to issues of clinical governance, quality and risk; participate in reporting incidents., • Work effectively with individuals in other agencies to meet patients' needs., • Maintain your DESMOND qualification and maintain your competency., • Undergo quality development as detailed in the ‘QD Pathway’ (available from DESMOND National Office) and professional development as required., • Demonstrate the ability to prioritise workload and to complete tasks independently with an ability to meet agreed deadlines., • Complete accurate records of work undertaken to comply with national, local and DESMOND standards., • Cover annual leave and short‑term absences by delivering the sessions independently in accordance with DESMOND guidance and the Trust lone‑working policy., • Band 3 level, • Accreditation as a DESMOND educator (or willingness to complete required training)., • Ability to work flexibly to meet service requirements under indirect supervision from the Team Lead., • Strong communication, organisational and team‑working skills., • Knowledge of short‑term behavioural change and diabetes self‑management principles., • Medical and public health literacy relevant to diabetes education., • Commitment to maintaining the highest standards of patient safety, clinical governance and quality improvement., • Experience in group facilitation and education delivery is desirable. #J-18808-Ljbffr
