Position Overview The Home Health Registered Nurse provides skilled nursing services to patients in their homes in accordance with physician orders and the patient’s individualized Plan of Care. The RN is responsible for performing patient assessments, coordinating care, educating patients and families, and ensuring compliance with Medicare, CMS, CHAP, and state regulatory requirements. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in the field. Key ResponsibilitiesPatient Care • Perform comprehensive patient assessments including OASIS assessments when required, • Develop, implement, and update the Plan of Care in collaboration with physicians and the interdisciplinary team, • Provide skilled nursing treatments and interventions as ordered by the physician, • Communicate patient status and changes to physicians and the care team, • Coordinate services with therapists, aides, and other healthcare professionals, • Maintain accurate and timely clinical documentation, • Ensure compliance with CMS, Medicare, CHAP, and agency standards, • Educate patients and caregivers on disease management, medications, and safety, • Supervise LPN/LVN staff and home health aides when appropriate, • Participate in meetings, training programs, and agency in-services, • Active Registered Nurse (RN) license in the state of employment, • Graduate of an accredited School of Nursing, • Valid driver’s license and reliable transportation, • Current CPR certification Preferred • Previous Home Health experience, • Experience with OASIS documentation, • Excellent clinical assessment and nursing judgment, • Strong communication and documentation skills, • Ability to work independently and manage time effectively, • Ability to travel to patient homes, • Ability to lift up to 50 pounds occasionally, • 401(k), • Health Insurance, • Dental Insurance, • Vision Insurance, • Life Insurance, • Paid Time Off 8:30-5
