Position Summary The Full-Time Hospice On-Call LVN is responsible for responding to urgent patient needs during on-call hours, providing symptom management, supportive care, and timely communication with the RN Case Manager and interdisciplinary team. Key Responsibilities • Respond promptly to urgent calls, including patient/family concerns, symptom changes, equipment needs, or caregiver support., • Perform after-hours visits for assessment, comfort care, and patient needs within LVN scope of practice., • Communicate patient condition changes to the RN Case Manager or on-call supervisor., • Assist with PRN visits such as medication administration, routine care, and safety checks., • Provide emotional support and education to caregivers regarding comfort-focused care., • Complete supervisory visits for hospice aides when assigned., • Document all care accurately and timely according to Medicare, state, and agency standards., • Participate in team communication to support continuity of care. Qualifications • Active California LVN license in good standing., • 1+ year hospice or home health experience preferred., • Strong assessment skills and ability to work independently., • Reliable transportation, valid CA drivers license, and auto insurance., • Ability to remain calm, compassionate, and professional during urgent or stressful situations., • EMR experience preferred (training provided)., • Reliable availability during assigned on-call hours.
