The Registered Nurse (RN), Mobile - Community Outreach Services is responsible for delivering patient-centered services following the TrueCare policies, procedures, and standards. The nurse plays a crucial role in reducing health care barriers for vulnerable population who may not access traditional medical settings. This role builds trust with vulnerable populations and works as part of an interdisciplinary outreach team to address acute, chronic and behavioral health needs. The Nurse role plays a key role in the implementation and delivery of immunization services in collaboration with the County Vaccine program. This position support vaccines access through community sites, school partnerships, outreach, education to support public health outcomes. Duties & Responsibilities: Nursing Functions: 80% • Performs routine patient care functions as per scope of practice following established clinical protocols, policies and procedures within TrueCare., • Administer vaccines following clinical protocols and safety standards., • Maintain, implement, and evaluate the vaccine program in accordance diverse programs such as Vaccine for Children (VFC), Vaccines for Adults (VFA) and private vaccines., • Manage vaccine inventory, storage and handling of the vaccines., • Ensure compliance with healthcare regulations, documentation standards and best practices., • Dispensed and/or administers medication orally or by injection as ordered by medical providers according to established policies and procedure and documents as proper., • Help providers or other medical staff personnel with examinations and treatments, as directed or needed., • Collaborate with providers and patients to assess urgency of patient problems, resolve questions concerning medications, health education, and medical advice within skill level and scope of practice., • Enter patient care, clinical data, and findings in the electronic health record (EHR) system., • Outreach and direct care: providing nurse care in various community setting such as health fairs, outdoor areas, vaccination events, in home visits., • Autoclave: oversight of process to ensure the cleaning, processing, sterilization and checking of instruments is in compliance to meet audit and infection control requirements., • Help nursing coverage during all hours of operation including weekends and coordination of medical services to all patients., • Collaborate with a multidisciplinary team to coordinate care and connect patients with necessary resources., • Shadow staff on skill competency as needed to ensure proper workflow processes and procedures are followed. Also, oversee and help in the training of new and existing staff., • Monitor and evaluate support staff on clinical skills competencies, as directed., • Maintain Title XXII up to date, the expiration update on all medications and lab supplies and discard per TrueCare protocol, when necessary., • Assist the Clinic Manager in the preparation for audits conducted by various agencies., • Performs preventive services such as vaccinations, screening (glucose, blood pressure, etc.), in the community events with or without mobile clinics., • Serve as a point of contact to support staff with questions, issues, or patient-related conflicts. When unable to address issues independently, bring them to the attention of the Clinic Manager., • Follow TrueCare policies and procedures for efficient department operations, reporting requirements, and supporting quality assurance program, safety, environmental and infection control standards., • Oversee/monitor and help in the training of new and existing staff., • Enhance medical visits by flipping the visits with provider which enables patients to be seen sooner than the next available appointment as needed., • Submit Confidential Morbidity Report to SDCA as needed.Educational Functions: 15%, • Provide vaccine information and education. Counseling patient on side effects, after care and future immunization needs., • Keeps up to date on policies and procedures changes and educates clinical staff (MA’s) about changes., • Support and take part in meetings and training seminars to continue education, knowledge, skills, and professional competence of the clinical staff., • Provides counseling and health education for patients and families when needed.Quality Assurance:5%, • Participate in audit readiness activity if needed., • Participates in ongoing Quality Improvement activities within the department., • Compliance with TC Title XXII, • Observes and documents competency of clinical staff (MA’S) and provides instruction and feedback on technique., • Monitor strict adherence to universal precautions as set up by TrueCare, DHS and OSHA., • Always Monitor Staff compliance with dress code and badge policy., • Monitor and evaluate support staff on clinical skills competencies, as directed.Additional Responsibilities:, • Maintains regulatory compliance of all back office/nursing activities (i.e., Logs, QC’s, and Nursing Stock Meds) and oversees compliance at designated sites., • Works as a collaborator and is willing to help other members with other duties within his/her capabilities and aids the clinic team in providing quality efficient patient care., • Maintains open communication with other clinic staff., • Maintains professional demeanor and excellence in customer service consistently with patients, caregivers, clinicians, and staff., • Participate in planning meetings, briefings, and debriefs related to clinic performance and public health updates.Qualifications:, • Graduate of an accredited California School of Professional Nursing., • Current California State Board RN license., • Current Basic Life Support (BLS) Certification for Healthcare Provider, • Computer ability, particularly with Electronic Health/Medical Record systems and the Microsoft Office suite, including Outlook, teams, and Word., • Availability to work a variable schedule, extended hours and/or Saturday shifts., • Possess the ability to work independently and as team member., • May be needed to travel between all TrueCare’s sites and locations therefore California Driver’s License, proof of valid insurance and clean driving record needed.Desired Qualifications:, • Bachelor of Science in Nursing (BSN) degree., • A minimum of 1 year of experience as a Registered Nurse., • Nursing experience in an ambulatory clinic and/or community health care setting. Experience administering vaccines, particularly in school, mobile or public health environments., • Bilingual in English and Spanish preferred but no needed., • Knowledge of vaccines schedules, VFC program, CAIR documentation system and CDC immunization guidelines.
