Job Description Job Summary The Hospice and Palliative Medicine Physician provides the medical component of the Hospice/Palliative Care Programs. The Hospice and Palliative Medicine Physician assists in the maintenance of appropriate medical policies and standards to meet agency, state, federal and accreditation requirements. This position will assist in providing hospice care and palliative care patients in various settings. The Hospice and Palliative Medicine Physician works collaboratively with the Transitions LifeCare medical directors and staff and acts as a liaison to the Physician Community. The schedule for this position is Monday-Friday 8:30am-5:00pm; On-Call rotation; weekends and holidays as requested. Responsibilities what you do • Provide hospice visits and palliative care consultation to patients in private homes, residential facilities, Hospice Inpatient Facility and hospitals as requested., • Provide consultative, supervisory and direct patient care utilizing pain and symptom management interventions that are based on established hospice and/or palliative care guidelines and standards., • Attend IDG meetings as appropriate, • Document in patient medical record to meet federal, state, regulatory, and agency guidelines., • Participate in activities that educate the medical community, hospital staff and community physicians about hospice, palliative care, pain, and symptom management, communication around end of life decision making and other Transitions LifeCare Programs., • Provide education to rotating students, residents and fellows., • Participate in physician on-call schedules to ensure that physician services are available 24 hours daily, 7 days per week., • Assist in data collection, forms, and outcome evaluation tools for the PalliativeCare Program as needed., • Provide support and oversight for advanced practice providers on the hospice and palliative care teams, • May serve as collaborating physician for advanced practice providers, • Participate in the development, review, and modification of comprehensive plans of care with other members of the patient care team., • Participate in the establishment and review of medical policies and procedures governing hospice/palliative care according to need., • In conjunction with the patient’s Attending Physician, certify patient prognosis at the time of hospice admission when needed., • Review/recertify patient prognosis for continuing hospice services on HMB/HMC patients as appropriate., • Be responsible for pharmacy utilization for patients under your care., • Attend weekly IDG meeting as a full and active participant. The Nurse Liaison serves as the first line point of contact for referral processing, admission coordination, and discharge planning in the hospital setting. In addition to referral coordination, the Nurse Liaison assists active TL patients with transitions of care in the hospital setting and acts as the hospice nurse case manager for active hospice patients admitted to the hospital under general inpatient level of care – this includes coordinating the hospice plan of care with hospital staff and hospice IDG members. The RN Liaison provides education and support to patients, families, hospital staff, and referral sources regarding end-of-life care and all TL services, by assisting with goals of care conversations and providing informational visits within the hospital setting as well as in other community-based locations as needed. Job Description Job Summary This is a full-time professional position. This position is responsible for coordinating nursing and hospice aide staff to ensure day-to-day continuity of care for all patients on service and ensure accountability of nursing care consistent with the agency’s mission statement and regulatory systems. This position supervises the assigned shift Hospice Home RNs, LPNS, and hospice aides. May intermittently include day to day supervision of all interdisciplinary team members. The schedule for this position is 40 hours per week; occaisonal weekends and holidays as scheduled. Responsibilities what you do • Coordinate interviewing/hiring and orientation of new clinical staff., • Support and assist with training of new staff, • Support and assist with medication delivery from pharmacy, • Supervise, direct, prioritize and coordinate delivery of patient care, on the assigned shift, for Hospice Home patients., • Ensure all admissions are assigned appropriately to clinicians, • Create a positive environment, fostering collaboration and a supportive atmosphere, • Demonstrate strong communication, problem solving, organizational and delegation skills, • Perform chart audits weekly to ensure compliance with documentation requirements., • Coordinate and problem-solve patient care issues with patients, families, physicians, agency staff and other health care providers., • Ensure staff documentation of patient care is complete, timely, and accurate in accordance with TL processes, policies, procedures and regulatory requirements., • Manage various personnel actions including, but not limited to hiring, orientation, performance appraisals, promotions and scheduling; and counsel and conduct formal discipline of clinical staff., • Develop and implement processes to enhance clinical operations and practices for ongoing quality improvement., • Oversee medication room operations and regulatory compliance related to narcotics and other medications., • Oversee clinical compliance with all regulatory and accreditation needs of the agency., • Review, update, and implement policies, procedures and clinical processes as appropriate., • Identify annual competency and staff development needs, in conjunction with PI and Education, ensuring all clinical staff are compliant with identified competencies., • Covers Management Back Up (MBU) as assigned, • Covers at least one holiday per year as assigned, • Attend seminars/workshops as appropriate., • Perform other duties as assigned by supervisor., • Professionally manages time, people, and technology.
