Job Description Christ Child House is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Residential Treatment Center serving youth who have experienced abuse, neglect, trauma, and behavioral health challenges. As a member of our treatment team, you will play a vital role in supporting the health, safety, and well-being of residents while working in a collaborative, trauma-informed environment. x The Licensed Practical Nurse (LPN) is responsible for providing nursing services to youth residing in a residential treatment setting. The LPN promotes the physical, emotional, and psychological well-being of residents through medication administration, health monitoring, medical coordination, and collaboration with the treatment team members. Qualifications/requirements: Education and Experience: Graduate of an accredited Practical Nursing Program. Minimum one (1) year nursing experience. Knowledge of medication administration and documentation requirements. Strong communication, organizational, and interpersonal skills. Preferred Experience: Experience working with children, adolescents, or behavioral health populations. Experience in residential treatment, foster care, psychiatric, or trauma-informed care settings. Licensure: Current Licensed Practical Nurse (LPN) license in the State of Michigan. Valid driver's license; limited moving violations in the past 5 years. Background Checks: All employees will undergo fingerprinting by the State of Michigan. Annual Internet Criminal History Access Toot (ICHAT), sexual abuse registry, and Central Registry (CR) check. Training: Nursing staff will receive 8 hours of orientation. Nursing staff are required to obtain 25 hours of training every 2 years with 2 hours of pain management and 1 hour of live/interactive implicit bias training for each year of the applicant's license or registration cycle. Physical: This position requires the ability to sit and stand for prolonged periods. This position requires the ability to stoop, squat, kneel and crouch. This position requires the ability to turn head and torso, bending and flexing arms, wrists and fingers, use of hands to feel objects, reach with hands and arms, and lift 25 pounds. This position requires computer literacy. This position requires full vision to read forms, reports, computer terminals, and printouts. This position requires the ability to operate a motor vehicle. This position requires full power of speech and the ability to utilize telephone, computers, and other instruments of communication with clients and employees. Working conditions: The Christ Child House makes every effort to provide a pleasant building atmosphere. This position may produce physical and mental stress. Person in this position must have the ability to respond quickly in emergency situations. They must also have the flexibility to work evenings, weekends, holidays, and on-call schedules as needed. Essential skills and abilities: • Model optimal personal and professional treatment standards, communicate professionally in oral and written form, and exercise independent professional judgment., • Ability to establish and maintain effective working relationships with co-workers and residents., • Ability to effectively communicate, verbally and in writing, with residents and with other staff., • Ability to transport residents, materials, and supplies safely and efficiently., • Positive interactions with youth, staff, and administration., • Assure safety of all residents including reporting safety issues and concerns to administration, supervisors/coordinators (building, staffing etc.)., • Present self as a positive role model., • Maintain a cooperative working relationship with other personnel., • Demonstrate ability to tactfully handle demanding situations., • Demonstrate cultural sensitivity in working with a diverse population., • Seek guidance and direction as necessary for performance of duties., • Maintain confidentiality of Christ Child House, client, and personnel information., • Ability to forge a mutually respectful partnership with persons served and their families in which they are helped to gain the skills and confidence to address any issues and problems they face., • Ability to successfully complete all post-offer of employment conditions including drug test and physical examination; free of communicable tuberculosis; able to obtain a central registry clearance., • It has a basic understanding of trauma, and the effects trauma has on our residents., • Employment will be terminated for any staff sleeping on the job. Staff are not allowed to sleep or mimic the posture of sleep. Essential duties and responsibilities: • Nursing Care, • Administer prescribed medications and treatments according to physician orders and established nursing protocols., • Monitor and assess residents' physical and mental health status., • Respond to routine and urgent medical needs of residents., • Provide first aid and basic nursing care as needed., • Monitor medication effectiveness and adverse reactions and report concerns to the physician., • Assist with health screenings, assessments, and medical appointments., • Monitor and assess height and weight routinely, • Be available for crisis calls, • Coordinate with specialists and doctors for health management and communicate information with staff, • Medication Management, • Maintain accurate medication administration records (MARs)., • Attend monthly medication reviews; Document MAR updates, scripts that need refills to psychiatrist and due dates for medications consents, • Ensure proper storage, handling, and disposal of medications., • Monitor medication inventory and coordinate prescription refills., • Maintain compliance with medication administration standards and agency policies., • Medical Coordination, • Schedule and coordinate medical, dental, vision, psychiatric, and specialty appointments., • Communicate with physicians, pharmacies, hospitals, and other healthcare providers., • Obtain and maintain required medical documentation and treatment orders., • Assist in developing and implementing resident health care plans., • Complete initial health screening and nursing consults within 24 hours of new admissions., • Complete medical discharge summaries and nursing consultations within 24 hours of discharge. 3.) Documentation and Compliance • Maintain accurate and timely nursing documentation in electronic health records., • Maintain resident medical files with up-to-date face sheets, medical appointments, psychiatric evaluations etc., • Complete incident reports and health-related documentation as required., • Ensure compliance with Michigan Department of Health and Human Services licensing requirements, Medicaid standards, HIPAA regulations, and Council on Accreditation (COA) standards., • Participate in quality improvement and accreditation activities. 4.) Team Collaboration • Participate in multidisciplinary treatment team meetings., • Provide health education and training to residential staff regarding medication administration, infection control, and health-related concerns., • Support a trauma-informed and therapeutic treatment environment., • Collaborate with clinical, residential, and educational staff to address resident needs., • Maintain your composure when dealing with non-compliant residents and request additional staff when necessary., • Attend extra staff calls and assist in crisis situations when a youth is restrained for over 15 Always used a trauma sensitive approach., • Always maintain safety., • Recognize and perform duties, which need to be performed although not directly assigned assisting others as needed. • Responsible for providing first aid to residents with minor injuries and caring for sick residents. • Maintain CPR/FA/AED certification and maintenance., • Responsible for performing all duties as assigned by Clinical Director, Director of Residential Services or Residential Supervisor, and/or other Manager.
