Job Description & Requirements • Specialty: Care Manager, • Discipline: RN, • Duration: 26 weeks, • 40 hours per week, • Shift: 8 hours, days, • Employment Type: Travel Job Summary: We are seeking an experienced Registered Nurse (RN) Case Manager to support care coordination and transition planning across the acute care continuum. The RN Case Manager will collaborate with physicians, social workers, utilization review teams, and interdisciplinary staff to ensure appropriate level of care, prevent readmissions, and facilitate timely and safe patient transitions. Key Responsibilities:Care Coordination & Transition Planning: • Coordinate patient care throughout the acute care stay, • Develop and implement cost-effective discharge and transition plans, • Collaborate with patients, families, physicians, and interdisciplinary teams, • Identify and resolve barriers to care progression and discharge, • Ensure timely transition to appropriate levels of careUtilization Management:, • Review patient charts to validate Level of Care (LOC) and documentation, • Monitor Length of Stay (LOS) and address outliers, • Collaborate with physicians to ensure proper documentation for reimbursement, • Maintain compliance with Medicare/CMS and Joint Commission standards, • Participate in patient rounds and escalation processes when neededPatient Assessment & Planning:, • Conduct comprehensive clinical, psychosocial, and financial assessments, • Identify high-risk patients and develop care plans to reduce readmissions, • Screen for supportive services including palliative care, • Coordinate post-acute care services and referralsCollaboration & Communication:, • Work closely with Physicians, Social Workers, HIM, Finance, and Nursing teams, • Communicate effectively with payers, community resources, and families, • 5+ years of RN Case Management experience, • Strong background in acute care hospital settingLicenses & Certifications:, • Active California RN License (Required), • BLS Certification (Required)Preferred Skills:, • Strong knowledge of care coordination, utilization review, and discharge planning, • Experience with EHR systems (Cerner/Epic preferred), • Excellent communication, critical thinking, and problem-solving skills, • Weekly pay, • Medical benefits
