Veteran Healthcare Navigator 35 hours per week / $25.00 - $26.00 per hour (The actual compensation will be determined based on experience and other factors permitted by law.) Summary: Under the supervision of the Program Manager or Program Director, the Healthcare/Housing Navigator provides Veteran-centered services focused on healthcare access, housing stability, and coordination of supportive services for Veterans and their families enrolled in the SSVF program. Chrysalis SECTOR Program Overview: Led by LA County’s Justice, Cares, and Opportunities Department, the Skills and Experiences for the Careers of Tomorrow (SECTOR) program is aimed at (1) reducing unemployment and increasing earnings; (2) reducing recidivism; and (3) improving behavioral health and well-being by preparing and matching people with justice involvement to jobs in high-growth industries. The goal is to significantly reduce the number of families in the child welfare system experiencing homelessness. The ideal BFH Housing Case Manager will work in collaboration with the Housing Locator/Matcher to network with owners, landlords and property managers to identify housing opportunities for homeless families, and to education them on UBH rental assistance programs. The BFH Housing Case Manager will also help educate on housing right laws and responsibilities, with the intention of mitigating any potential tenant-related matters, including deterrence of evictions. Our mission is to meet our participants’ needs through an individualized and non-judgmental approach that will ultimately lead to long term housing stability. The Substance Use Counselor will provide counseling and support to clients suffering from drug or alcohol dependency, psychoeducate clients on evidenced based practices to help in their recovery process, assist with case management needs, and work on linking clients to Medication Assisted Treatment, detox, and residential services as needed. Under the supervision of the clinical supervisor, the Substance Use Counselor will assist in conducting seeking safety, harm reduction, trauma-informed, low-barrier, culturally responsive, and person-centered care to improve mental health and behavioral health outcomes of participants residing at Hope the Mission’s interim housing shelters. This position will be a part of a multi-disciplinary team of staff including but not limited to clinical supervisors, clinicians, mental health care coordinators, substance use counselors, and peer support specialists and provide support to staff in other departments through case consultation, training, and de-escalation.
