Job Description The Registered Nurse Home Health Case Manager, at the request of the Intake Department, provides nursing services to patients as ordered by the physician. The nurse is a specialist in assessing the patient's condition and developing a plan of care to improve, monitor, and document the course of treatment prescribed by the physician for the patient's diagnosis. The plan of care is developed to reduce symptoms, educate the patient, promote compliance with the prescribed plan of care, and assist the patient in becoming independent with self-care when capable, based on the individual needs of the patient. Additionally, the nurse communicates with all members of the care team to ensure proper coordination of care. Job Summary The Housing Specialist provides comprehensive direct services to individuals and families facing housing instability. This includes identifying housing resources, navigating homeless services, and assisting with rental and utility assistance programs. The role also serves as a key support for clients residing in subsidized housing, helping them maintain stable housing, set goals toward permanent housing, and monitor progress. The Housing Specialist works collaboratively with clients and service partners to promote housing security and long-term self-sufficiency.
