About this Position: The Community Health Centers of Lane County is seeking a Family Nurse Practitioner to be part of an interdisciplinary team that provides comprehensive primary care within the Patient Centered Medical Home model. This mission driven individual will work as part of an interdisciplinary team at our South Lane Clinic.
