BioMatrix Specialty Infusion Pharmacy is seeking a Part-Time Per Diem Registered Dietitian to see patients in their homes in Syracuse, NY. Work location is subject to change based on business needs. On-Call: Participation in the on-call rotation is a requirement of this position. It is anticipated that an incumbent in the position would be on-call approx. once every eight (8) weeks for two (2) weekdays per month and one (1) weekend. The Per Diem Registered Dietician collaborates with Upstate Home Care clinical teams to ensure optimal nutrition support for patients receiving enteral or parenteral therapy. This role supports qualification of TPN patients, secures appropriate clinical documentation, ensures compliance with insurance guidelines, and provides clinical expertise as needed. Additional responsibilities include assisting with nutrition support services, maintaining formularies, contributing to policy and procedure development, participating in staff education, and supporting reimbursement efforts related to nutrition therapy. QUALIFICATION REQUIREMENTS • Bachelor’s degree in nutrition or related field., • Minimum two years of professional nutrition experience., • Minimum one year of experience in a home care setting., • Current, active registration as a Registered Dietitian (RD) by the Commission on Dietetic Registration (CDR)., • Current, valid state licensure or certification to practice dietetics., • Certified Nutrition Support Clinician (CNSC) through the National Board of Nutrition Support Certification, Inc. QUALIFICATIONS PREFERRED • Experience working with parenteral and/or enteral nutrition therapy in home infusion or home care environments., • Prior involvement in interdisciplinary care teams., • Familiarity with payer guidelines and reimbursement requirements for nutrition support services. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Clinical Support • Perform competent clinical assessments and maintain required licensure and continuing education., • Monitor written orders for specialized nutrition therapy and collaborate with pharmacy staff to ensure accuracy., • Evaluate patient clinical status, effectiveness of nutrition therapy, and appropriateness of ongoing treatment., • Maintain compliance with Upstate Home Care policies, procedures, and documentation standards., • Demonstrate sound clinical and business judgment aligned with organizational goals., • Foster positive working relationships with prescribing physicians and cross-functional teams., • Assist in developing parenteral nutrition training materials and reference guides for Upstate staff., • Support the acquisition and review of qualifying documentation for referrals., • Uphold organizational mission, payer regulations, and confidentiality requirements., • Adhere to complaint, incident, and exposure reporting guidelines., • Understand and comply with Do Not Resuscitate (DNR) policies and related responsibilities., • Assist colleagues when needed and support team collaboration., • Stay informed on newly introduced products and services related to nutrition therapy., • Complete required training, testing, vaccinations, and in-service education., • Act as a patient advocate while maintaining confidentiality and safety., • Promote dignity, respect, and cultural sensitivity., • Apply principles of beneficence, non-maleficence, autonomy, justice, and veracity in clinical practice., • Use company resources responsibly and present honest, accurate reporting of expenses., • Manage time effectively, prioritize appropriately, and follow through on commitments., • Communicate with professionalism, using clear, respectful, and inclusive language., • Answer calls promptly and return messages within established expectations., • Respond calmly to patient or caregiver concerns and support conflict resolution without escalating unnecessarily., • Represent Upstate Home Care positively in all interactions with patients, partners, and colleagues. NON-ESSENTIAL FUNCTIONS & RESPONSIBILITIES • Participation in community, referral partner, or educational presentations as assigned., • Occasional support for cross-departmental administrative projects., • Other duties as delegated to support clinical operations. KNOWLEDEGE, SKILLS AND ABILITIES REQUIREMENTS • Strong clinical knowledge of parenteral and enteral nutrition therapy., • Ability to interpret clinical documentation, lab values, and nutrition assessments., • Strong written and verbal communication skills., • Excellent organizational skills with attention to detail and accuracy., • Ability to work independently and collaboratively within a multidisciplinary team., • Competence in time management and prioritization in a fast-paced environment., • Knowledge of payer guidelines and documentation requirements for reimbursement.
