This can be structured as either three 8-hour days or four 6-hour days, based on your preference. The role will work primarily at the Meng Residence in De Pere, with occasional community coverage as needed. One holiday per year is required. Knowledge, Skills, and Abilities: • Computer proficiency (MS Office – Word, Excel, and Outlook), • Highly motivated and organized with a strong attention to detail and analytical skills to help solve problems., • Ability to communicate effectively, both verbally and in writing., • Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment., • Ability to keep up to date with policies, practices, and regulations., • Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures., • Comply with all applicable local, state, and federal laws and regulations as well as policies and procedures of Unity., • Satisfy mandatory training requirements regarding local, state, and federal regulations and/or Unity’s policies and procedures., • Perform other job-related duties as assigned., • Ability to adjust to frequently changing demands. What you'll do: As a Primary Social Worker, you will play an essential role on our interdisciplinary team, supporting patients and families through the end-of-life journey. Your work includes: Providing direct patient care support at the Meng: • Complete initial psychosocial assessments, • Develop a plan of care addressing the psychosocial needs of the patient, family and caregivers, • Update the plan of care with changing patient/family needs, • Provide emotional support to patients and their families, • Engage in ongoing communication with patients, families, facility staff and team members to expedite the plan of care, • Complete comprehensive assessments based on patient/family need, • Coordinate and participate in patient and family care planning meetings, • Exhibit flexibility and creative problem-solving for patients with complex psychosocial needs, • Serve as a patient advocate. Enhance a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions and be an active member of the interdisciplinary team, • Serve as a resource and provide education to patient, family, facility staff, physicians, and professional staff relative to the psychosocial needs of patients and families, • Assess the family unit of care for bereavement risk throughout care, • Attend IDT meetings, • Participates in the development of the initial plan of care, and provides input into the review and revision., • Serves as a resource to other patient-care staff., • Consistently and concisely document activity according to department protocols and standards, • Make referrals and coordinate care with local agencies to ensure patient has needed supports, • Assist in planning funeral services, prior to death, as requested by patient’s family, • Assist in oversight of the Meng waiting list., • Assist in arranging transportation to the Meng Residence or discharge to home or another facility., • Participate in daily rounding at the Meng when needed. Other: • Foster positive internal and external customer relations, positively representing the social work profession throughout the organization and in the community, • Adhere to the Social Work Code of Ethics and consistently practice within a framework guided by ethical principles, • Perform other job-related duties as assigned, • Maintain regular and reliable attendance, • Participate in community education and networking opportunities as needed, • Shares in providing holiday coverage as needed, • Assists in training and welcoming volunteers to the Meng Residence., • Work with volunteer services department to place volunteers with patients to provide socialization and companionship. Also, coordinate Veteran pinning ceremonies for patients and contracted facilities., • Participates in process improvement projects to enhance the psychosocial services provided to patients and families., • Utilize Unity’s specialty programs to enhance patient care. (Ex. Gifted Wishes, Holistic Harmonies, Respecting Valor), • Comply with all applicable local, state and federal laws and regulations as well as policies and procedures of Unity, • Satisfy mandatory training requirements regarding local, state and federal regulations and/or Unity’s policies and procedures, • Consistently and concisely document activity according to department protocols and standards, • Promote and maintain confidentiality, • Maintain personal and professional boundaries Why Unity? Requirements: • Bachelor’s in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or bachelor’s degree in psychology, sociology, or other fields related to social work along with one year in a healthcare setting. Master’s of Social Work preferred., • Certified or licensed as a social worker (CSW, CAPSW, CISW, or LCSW) by the State of Wisconsin required., • Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
