The Health Services Patient Advocate is responsible for the maintenance of relationships with the 5 Star Centers, supporting the operations of the Health Fund Programs, responding to member and provider issues, and assisting members in choosing in-network providers, particularly 5 Star Centers. The Health Services Patient Advocate works on issues from initial inquiries through completion and participates in marketing strategies and provide feedback for continuous quality improvement of the Health Services Team. Additionally, this position provides other operational support activities within the Health Services Team. Principal Duties and Responsibilities: • Develop relationships with management and administrative staff at assigned 5 Star Center, including site visits, • Assist members in choosing providers and scheduling appointments at 5 Star Centers, • Assist with issue resolution as problems arise for assigned 5 Star Centers from initial inquiry through completion, • Log member issues into the department tracking system, • Assist members and act as advocates during complaint process with providers, • Identify any patterns of fund program issues with service provision and address accordingly or notify management, • Address member concerns about Fund programs, • Complete all program enrollment processes for eligible, interested members, • Utilize Health Services Database to track interactions related to Fund programs, • Work with members and the Providers to determine the correct copay and rectify all billing errors, • Reach out to members and dependents who meet specific criteria for upcoming Fund initiatives and or programs, • Receive incoming calls through the Health Services queue regarding Health Services Programs, • Back up member services provider line queue upon request for provider search activity, • Attend and participate in all team meetings Qualifications and Core Competencies: To perform the job satisfactorily, an individual must be able to perform each essential duty satisfactorily. Job Description Overview Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Responsibilities Under supervisor of the Sited Medical Director with latitude for independent judgement and initiative, the Physician Assistant is an integral part of the medical team at his/her facility. He/she is expected to take an active role in the delivery of health care services to our patients, and relationships with multiple training institutions throughout the city. Under the supervision of the Vice President of Social Responsibility the Site Manager is responsible for providing over site of all operations and administrative functions for the Code Blue service. The Code Blue Site Manager assures safe and smooth operations of the program participants, manages/supervises staff, and collects data, while maintaining consistent communication with the referral sources and program partners Requirements: The Code Blue Site Manager should have an understanding of Center procedures and be willing to learn the building system and the details of the Code Blue operations. Must be able to relate to an extremely diverse population of people, be flexible and work as part of a team. Excellent interpersonal skills are required. The employee must display a professional and patient personality. The Code Blue Site Manager must be dependable at all times regarding shifts.
