Job Title: Field Nurse Practitioner Location: Brooklyn, NY Work Mode: Hybrid (On-site with some remote work available) Employment Mode: Permanent (Full-time) About the Role We are seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to deliver home-based primary care services in Brooklyn region, NY. This role is ideal for a provider who is passionate about improving health outcomes through personalized care in the home or assisted living setting. You will manage a panel of patients and address both acute and chronic conditions in collaboration with a supervising physician.
