This position is also responsible for the ICHS clinics patient navigators who act as part of the care team and assist patients with addressing social determinants of health (SDOH). The Community Services Manager will work in collaboration with the Community Access Manager to develop and implement services at ICHS’ community partners’ sites. This position will manage staff providing services at multiple sites and community settings, requiring knowledge of multiple communities served. This position will work with managers, supervisors, administrators and leads across the department and organization to ensure compliance and alignment with clinical practices at ICHS’ community based clinics. This position represents the organization to a variety of community partners including school districts, school administrators, Seattle King County Public Health, Neighborhood House, Eastside Pathways and many others. Education - BA required; MSW, MPH, or related Masters preferred.Two (2) years of college completion and/or Associates Degree with additional four (4) years of progressively responsible leadership positions may substitute for the required degree.
