One such organizational change is the creation of a new division called SAGEServes, housing the agency’s New York service programs and older adult centers. Summary: The Director of Care Management Services is accountable for all aspects of SAGE’s Care Management programming throughout New York City, including contract management and oversight and supervision of 12 department staff.
