Essential Functions: Describe the intrinsic job duties that define the core responsibilities of the job.  Provides advocacy, support, and bridging services with a focus on wellness and resiliency to participants with mental illness to help improve their quality of life.  Inspire individuals to become more hopeful about their recovery.  Provide advocacy on behalf of the program participants in line with the choices of the individual being served.  Explains the types of services and programs available to participants as it relates to their stated goals or interests.  Peer Advocates serve as facilitators of self-help groups and wellness services to assist participants address their whole health needs, develop coping strategies and connect with critical resources such as housing, education and employment.  Record/document services provided as required by program and regulatory agencies  Peer Advocate functions as a model, sharing his or her own stories to therapeutically support program participants and their families.  The Peer Advocate strengthen the individual by creating an atmosphere of understanding, assisting in skill development, broadening their awareness, opening up new opportunities, and increasing their sense of hope, personal power and self-responsibility.  Help remove stigma and barriers associated with moving on to more independent roles  An important function of peer support is to reduce the possible stigma and personal effects of having a diagnosis of mental illness by sharing their own experience.  The Peer Advocate will role model competency in recovery and on-going coping skills.  The Peer Advocate serves as a bridge to connect to the community by helping individuals begin to move beyond strictly professional support to that of peer and community through The position may require the performance of other essential and non-essential job duties in addition to those enumerated above which will be determined at the discretion of management. Job Description WEST SIDE FEDERATION FOR SENIOR AND SUPPORTIVE HOUSING Social Worker I (Bilingual /Spanish) The Social Worker I works as part of the social service team. They report to the Clinical Coordinator, or Director of Clinical and Social Services, while working closely with their Building Manager. A Social Worker I is an exempt employee who is required to have a Masters’ Degree. They are expected to be able to work autonomously while developing 1:1 relationships with the residents. This relationship will enable the Social Worker I to provide a comprehensive range of social work services to the residents and enable the residents to establish and maintain safe and independent lifestyles in the community. WSFSSH strongly supports M.S.W. level social workers obtaining their licensing. We offer study groups for staff who are preparing to take their exam. WSFSSH will also pay for a study course if needed, and pays for up to 2 licensing exams, as well as registration/licensing fees. Once a MSW staff member has obtained their LMSW, WSFSSH will cover reasonably priced ceu credits in order to meet licensing requirements. In addition, WSFSSH will provide weekly clinical supervision by a LCSW, and because WSFSSH is a waivered agency with the NYS Education Dept, a LMSW's work experience will count towards their LCSW. Typically, after 3 years of full time employment, a LMSW will be eligible to apply for their LCSW. You Will: • Conduct inpatient coding audits on medical records, utilizing ICD-10-CM, CPT, and appropriate coding references for accurate DRG and APC assignment., • Review non-CC/MCC records to assess proper coding or identify the need for additional documentation. Scrutinizes all HCPCS and CPT codes influencing APC assignment., • Provide coder education through the auditing process., • Prepare preliminary results for review by the facility or CCS HIM director., • Review disagreements on APC/DRG changes with the appropriate manager., • Prepare the final reports for the coding audit and actively participates in the resolution of audit findings., • Provide coder education via email and/or conference calls, utilizing the audit spreadsheet findings and comments., • Attend coding workshops as necessary., • Stay current with regulatory changes., • Organize and prioritize multiple cases concurrently to ensure departmental workflow and prompt case resolution., • Demonstrate versatility and exceptional work across a wide range of coded services., • Meet with client facility representatives to discuss issues and trends identified in audits., • Develop and implement education for physicians, nursing, and other clinical staff to enhance documentation., • Communicate effectively with co-workers, management, and hospital staff regarding clinical and reimbursement issues., • Function in a professional, efficient, and positive manner., • Adhere to the American Health Information Management Association's code of ethics., • Maintain a customer-service focus and exhibits professionalism, flexibility, dependability, a desire to learn, commitment to excellence, and commitment to the profession., • Conduct audits on external coding staff as needed and provides reports to the manager as directed., • Handle a high complexity of work functions and decision-making.
