In this varied and rewarding role youll be the welcoming face of the ward supporting patients families and staff with the day-to-day running of the unit. Youll also play a key part in coordinating patient discharges making sure that everything is in place for a smooth and stress-free transition home or into community care. About the role: The Ward Clerk & Discharge Liaison plays a key role in supporting the efficient running of our Inpatient Unit. This position provides essential clerical and administrative support to the clinical and multidisciplinary teams ensuring the smooth coordination of daily ward activities and the effective management of patient admissions and discharges. A central aspect of this role involves facilitating patient discharges. The postholder will liaise closely with Continuing Healthcare (CHC) teams general practitioners nursing homes and other external partners to ensure all documentation is accurate complete and processed in a timely manner. The role also includes maintaining patient records managing telephone and email enquiries arranging transport and supporting the wider team with administrative duties to enable the highest standard of patient care. About the person: An organised reliable and compassionate individual who demonstrates excellent administrative and communication skills. You should be confident using IT systems and managing multiple priorities within a busy healthcare environment. Previous experience in an administrative or clerical role particularly within a healthcare or patient-facing setting is desirable. You will have a professional and empathetic approach maintaining confidentiality and sensitivity in all aspects of your work. Key responsibilities: • Provide an efficient and welcoming reception service for patients families and visitors., • Support the smooth running of the ward through accurate and timely administrative tasks., • Manage patient admissions and discharges ensuring records and documentation are complete and up to date., • Liaise with Continuing Healthcare (CHC) teams GPs and community services to coordinate patient discharges., • Maintain and update patient information on the electronic record system (EMIS)., • Handle telephone and email enquiries promptly and professionally., • Arrange transport for patient transfers appointments and discharges., • Ensure ward paperwork forms and supplies are well organised and replenished., • Communicate effectively with the multidisciplinary team to support efficient patient flow., • Salary from 24625 up to 26187 per annum (FTE) dependant on experience, • Working hours are 30 per week working days Monday Tuesday Thursday Friday with future flexibility to cover weekends., • Office location: IPU Brentry, • Access to staff wellbeing and employee support services Due to the nature of the work involved this role is exempt from the ROA and thejobholderwill berequired to undergo a Disclosure and Barring Service Check.
