As a Director of Clinical Services (DOCS) with 360 Behavioral Health, you use and build upon the expertise you’ve developed as a Board-Certified Behavior Analyst (BCBA). At this point in your professional journey, you join our clinical leadership team and you “own” your clinical site and take responsibility for the site’s entire caseload. Job Description WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. The Personal Care Assistant is a vital part of our team, providing aid to our participants as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Day Center Manager, the Personal Care Assistant will provide safe delivery of personal care services to participants at the WelbeHealth day center. Job Description WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. The Assistant Clinical Nurse Manager is accountable for both people management and RN-level nursing care. Under the direction of the Executive Director, the WelbeHealth Assistant Clinical Nurse Manager directs the day-to-day work at the clinic, oversees the clinic staff, and coordinates clinic flow to ensure effective WelbeHealth clinical services are provided, and quality of care standards are met. Essential Job Duties: • Provide leadership, oversight, and performance management to RN, LVN, and MA roles, ensuring all have role clarity and required training to perform their functions at the top of their license, • Oversee clinic function and clinical needs in the center to ensure chronic and acute needs of participants are addressed properly, as well as liaise with the Complex Care team on smooth transitions of care to meet post-discharge needs, • Implement and enforce workforce plans and ensure appropriate staffing, recognizing the diverse experience levels of staff members and the unique needs of participants, • Participate actively in direct participant care to ensure needs are met, practicing within the RN level of care, • Effectively intercede with participants and their families, resolving concerns related to the delivery of health care services, • Graduate of an Accredited School of Nursing with unencumbered RN license with which you practice required; bachelor's degree (BSN) preferred, • Five (5) years of nursing experience in a clinical setting with a frail or elderly population preferred, • Two (2) years of supervisory experience with demonstrated aptitude to mentor and develop team members preferred, • Excellent leadership, organizational and communication skills in settings with seniors, their families, and interdisciplinary team members, • Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects, • 401k + employer match, • Medical insurance coverage (Medical, Dental, Vision) starting day one of employment, • Work life balance – we mean it! 17 days of personal time off (PTO), 12 holidays observed annually, 6 sick days, • And additional benefits Salary/Wage base range for this role is $132,181- $174,478 / year + Bonus + Equity.
