Job Description Job Title: Street Outreach Case Manager Location: 1919 NW Loop 410 Reports To: Street Outreach Program Manager FLSA Status: Exempt Approved By: Rex Brien, January 2026 Salary: $42k-$45k Annually Apply online: SAMMinistries’ Street Outreach Case Manager will provide case management support to adults and children who are currently experiencing homelessness. The Street Outreach Case Manager is responsible for connecting with, and engaging, the homeless population on the streets of the greater Bexar County area and linking them to services. In addition to visiting known places where individuals experiencing homelessness congregate, the Street Outreach Case Manager works collaboratively with other service providers to ensure client needs are met. Including but not limited to the City of San Antonio abatement and assessment process. The population being served includes individuals meeting HUD’s definition as literally homeless, chronically homeless, and/or disabled. The Street Outreach Case Manager’s focus is on providing services to SAMMinistries’ clients that support development of skills to foster long-term housing stability and self-sufficiency. Essential Job Functions and Responsibilities • Connect with and engage with clients and conduct assessments needed to enroll families/individuals into Coordinated Entry System for homeless services. Connect clients to mental health and substance abuse treatment, and crisis intervention as needed. Communicate assessments, interventions, and ongoing client contact through timely, thorough record keeping, contact forms, and verbal communication., • Actively engage with clients on active caseload to ensure they are connected to resources that address problems underlying the current/recent experience of homelessness., • Advocate for the needs of individual clients as well as for systemic change, whenever needed for the overall benefit of homeless individuals., • Attends and actively participates in team meetings, demonstrating an ability to prioritize participant needs and provide needed assessments and services in an ethical manner., • Identify, develop linkages with, and coordinate access to services in the community and ensure active follow-up occurs. Work collaboratively with staff of other community agencies to establish and maintain partnerships that maximize benefits and resources for SAMMinistries Program participants., • Transport SAMMinistries Program participants, as needed. This could be in a personal vehicle., • Provide crisis intervention for program participants, including crises involving mental health, assessing for potential danger of participants harming themselves or others, assessing for danger in situations involving domestic violence, etc., • Complete necessary documentation and tracking of activities, in accordance with all agency and grantor requirements., • Understand and maintain client confidentiality and record-keeping, in accordance with HUD, SAMMinistries, and HIPAA privacy practices., • Adhere to strict boundaries and professional ethics in the care of others., • Enter data into HMIS in a timely and accurate manner., • Be available to provide crisis intervention after regular business hours, including but not limited to inclement weather response and other emergency situations., • Perform other duties, as assigned. Page Break Mandatory requirements: • Employment eligibility verification, • Reliable transportation, valid driver’s license and auto insurances, • Successful completion of background screening, • This position requires on-site face to face contact with clients and program staff. Supervisory Responsibilities None Education and/or Experience • Bachelor's degree in social work or related field., • Minimum two years of experience working with individuals experiencing homelessness preferred, or a minimum of two years’ experience working with vulnerable populations in a closely-related field., • Must understand and use approaches and models that are aligned with SAMMinistries Rapid Re-Housing, Prevention and Permanent Supportive Housing Programs, including Housing First and Trauma Informed Care. Experience with principles and approaches of Harm Reduction and Motivational Interviewing., • Must expand and improve case management skills through formal training, reading and on-the-job experience., • Must be able to prioritize multiple job responsibilities to produce a high volume of work in a short period of time., • Must communicate orally and in writing in a professional, clear, and accurate manner., • Must possess excellent interpersonal skills and be able to work with project participants, agency staff, and community partners, productively. Language Skills Bilingual (English/Spanish) is preferred. Mathematical Skills Ability to calculate figures and amounts such as addition, subtraction, division and percentages. Certificates, Licenses, Registrations Class C driver's license. Licensed social worker or equivalent licensure desirable. Work Environment The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene. The employee must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure. May be required to work flexible hours to include evenings and weekends.
