Health Navigation is the process of advocating for and guiding a patient through the medical and behavioral health system. We provide one-on-one assistance with appointments, medications, and health literacy. The average case load is approximately 15-25 patients per Navigator, based on the level of care and service required for the patients. Gellert Health provides our patients with holistic and patient-centered assistance to access the care they need, which may include finding a healthcare provider, attending medical appointments together, understanding insurance coverage, and helping the patients better manage their medications and chronic conditions. By providing targeted support and guidance, we help our patients access timely and quality care. The ultimate goal of our program is to help patients receive the best possible care, and to improve their well-being.
