About the Regional Health Connector Program The Regional Health Connector (RHC) Program is an innovative workforce, dictated by statute, that supports systems change and clinic-community linkages across Colorado. RHCs focus on changing systems by collaboratively improving health-related social needs, responding to emergent issues (such as the COVID-19 pandemic), and helping support or lead locally tailored priority projects that address health issues in their communities. Position Summary The RHC Program Manager will manage and further develop the implementation of the Regional Health Connector program. The RHC Program Manager will report to the Senior Director of Programs and will work in collaboration with the Regional Health Connectors (RHCs), host sites, the University of Colorado Department of Family Medicine (CU), and the Colorado Department of Public Health and Environment (CDPHE). For more about the program structure and organizations involved, click here. This position will work a hybrid-remote schedule with a focus on Colorado communities. Responsibilities: • Act as a primary liaison between RHCs and state entities like CDPHE and CU and ensure clear, consistent communication of program expectations, strategic goals, and available resources., • Provide support and guidance to RHCs and CU for completion of program deliverables., • Engage in the development and implementation of a new strategic plan for the RHC program., • Facilitate effective communication and information sharing across the RHC workforce to reduce silos and foster a cohesive approach to community-clinical linkages., • Develop and implement clear accountability measures and hold RHCs responsible for the work outlined in legislation and in their contracts and scopes of work., • Facilitate engaging and educational monthly meetings with the RHCs via web-based platforms., • Provide technical assistance to the RHCs and the host organizations where they are housed., • Develop and facilitate trainings, including an annual work retreat for the RHCs., • Develop and implement comprehensive onboarding processes for RHCs., • Work directly with and manage accountability of host organizations to ensure their understanding and support of RHC program performance and contractual obligations., • Develop communications and messaging between Trailhead and programmatic partners, in collaboration with the Trailhead Communications Team, including updates to the RHC website., • Assist in the maintenance of the RHC program budget with support from the Senior Director of Programs and finance team at Trailhead., • Work collaboratively with Trailhead's programs team to build relationships and cross-coordinate shared program priorities., • Reference and integrate Trailhead's strategic plan in all program development and decision making. Desired Experience • The successful candidate will be a self-starter who understands the value of collaboration and the ability to communicate across the organization and with community partners., • Previous experience and/or knowledge of the Colorado Public Health system is helpful., • Minimum 3-5 years of experience in program implementation or management., • Strong leadership skills with the ability to guide, collaborate with, and motivate a diverse workforce., • Interest or experience in public health., • Excellent relationship building skills, ability to work with a variety of internal and external stakeholders and community partners., • Excellent interpersonal and conflict resolution skills, able to resolve issues directly as they arise through the practice of ethical communication., • Excellent communication skills, both written and verbal, with the ability to translate complex strategic concepts into clear, actionable guidance for diverse audiences., • Demonstrated experience using and developing reports and messaging that are clear, cohesive, actionable, visually compelling and easy to understand., • Proficiency in Microsoft Office, Outlook, Zoom, Slack, Adobe, as well as the aptitude to learn new computer applications as needed., • Keen analytic, organization and problem solving/decision-making skills., • Ability to troubleshoot, take initiative, be forward thinking in Trailhead's program areas., • Demonstrated work history of coordinating and completing tasks independently and on time. This is a full-time, exempt position with office facilities in Denver, Colorado. This is a hybrid-remote position, with quarterly in-person meeting attendance requirements. The applicant selected for this position must reside in Colorado. All business is conducted in Mountain Time. Salary & Benefits Salary range for this position is $74,000 - $80,000. Trailhead Institute offers a generous benefits package including paid vacation and sick time (prorated for part-time employees), health, dental, vision, life, short-term and long-term disability, a free RTD EcoPass, and a 401k with a 100% employer match up to 10% of compensation. This is a LIMITED SERVICE position. The funding for this position is provided by limited-term funding and is dependent on that funding being maintained. Once the funding ends, the position will be discontinued, and there is no guarantee for additional employment with Trailhead Institute. Full time employment is anticipated through June 2029. This position is dependent on funding from state sources and subject to change. License and Proof of Insurance Requirement If you will be using a personal vehicle to drive yourself for Trailhead Institute business, you must provide a valid driver license and proof of insurance. While access to reliable transportation is a must, having a driver's license and/or personal vehicle is not a requirement for employment with Trailhead Institute.
