Job DescriptionDescription: SUMMARY The Substance Use Recovery Specialist will implement substance use recovery programing for dually diagnosed adults living in ACOF’s various apartments. This position will coordinate services practicing harm reduction and housing first principles, including leading on-site support and recovery groups and 12 step meetings or coordinating these groups through service sub-contractor, consulting with supportive services and property management staff regarding tenants’ progress and participation; making referrals with clinicians, or impatient addiction counseling centers as appropriate. The Specialist will also continue to establish relationships with clinics, substance use recovery programs, treatment centers, community associations and coalition meetings. This position provides Intensive Case Management Services (ICMS) to formally homeless individuals and families with chronic medical conditions and co-occurring disorders, who are high utilizers of the Department of Health Services (DHS) hospital and outpatient system. Under the direction of a Tenant Services Supervisor, the Case Manager I (CM-I) is responsible for providing individual case management to formerly homeless individuals and families utilizing a theoretical framework incorporating Motivation Interviewing (MI), Critical Time Intervention (CTI) and Stages of Change (SoC). This framework provides the CM-I (ICMS) with tools that reflect best practices to motivate tenants to achieve their goals in a non-judgmental manner. The CM-I (ICMS) will perform specialized tasks including comprehensive case management services such as intake, assessment, goal setting, monitoring and reassessment, life skills, counseling, individual benefit assistance, referrals and linkages to all tenants. The CM-I (ICMS) will document all tenant interactions and input that data into the Homeless Management Information System (HMIS) and the CHAMP (DHS) database. This framework provides the CM-I with tools that reflect best practices to motivate tenants to achieve their goals in a non-judgmental manner. Casa Community, a division of Casa de la Familia, is looking for victim advocates for our Domestic Abuse Response Team (DART)! This is a unit that partners specially trained officers with certified domestic violence counselors to provide immediate crisis intervention, trauma-informed care, and follow-up services to survivors and their families at domestic violence calls. The Senior Case Manager II will mentor the team, and, as directed by the Program Coordinator, will arrange, coordinate, monitor, and assure all delivered services by the case management team meet the needs of clients. This includes assisting case managers in: assessing and reassessing client/household needs, educating on program and community resource opportunities, developing relevant service plans, scheduling appointments, and providing necessary follow-up to ensure service plans are progressing on schedule and needs are adequately being addressed. The position requires a combination of administrative/office duties and field/outreach work.
