Certification Details • BLS, • CRRN Med/Surg, • NYS RN LicenseJob Details, • Clinical Nurse is a Registered Professional Nurse providing safe, competent quality care based on nursing theory and research to a designated group of patients and significant others., • Delivers patient care incorporating Relationship Centered Care and nursing process based on theory, research, evidence-based practice, and organizational nursing standards., • Assesses and evaluates patient care needs applying critical thinking skills in patient care management., • Integrates relevant assessment and intervention skills in nursing care delivery., • Collaborates with interdisciplinary healthcare team in plan of care development, implementation, and evaluation., • Prioritizes patient care including teaching, rounding, coaching, planning after hospital care, and delegating as appropriate., • Educates patients and caregivers about plan of care, transition of care, health promotion, and disease prevention., • Communicates effectively and professionally with patients, family, and interdisciplinary team members., • Manages assignments within Modified Primary Nursing Care Delivery Model and allocates material resources effectively., • Role models Mount Sinai Relationship Centered Care through various patient care actions such as Admission Welcome, Bedside shift Report, HELP AM/PM Care, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration., • Considers patient's values, preferences, cultural diversity, expressed needs, and knowledge in care., • Establishes and maintains therapeutic relationships with patients and families., • Uses evidence-based practices to understand patients' perceptions of care., • Initiates service recovery for patient and family concerns and escalates as needed., • Demonstrates caring, respect, compassion, empathy, and active listening., • Supports staff use of evidence-based practices to understand patients' perceptions of care., • Conducts Purposeful Hourly Rounding addressing the 4 Ps., • Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement., • Demonstrates clinical skills and knowledge crucial to quality and safety in patient population served., • Implements process improvement strategies based on institutional, evidence-based procedures., • Engages in peer and institutional review processes., • Collaborates with interdisciplinary teams on quality improvement projects, evidence-based practice activities, and nursing research studies., • Contributes to quality and safety practices and compliance measures., • Implements principles of high reliability to avoid high risk behaviors and provide a safe environment., • Escalates potential safety hazards or gaps following institutional chain of command., • Utilizes institutional information technology for nursing documentation supporting quality and performance improvement., • Performs charge nurse duties as assigned., • Coordinates with management to ensure appropriate supply utilization and inventory for patient care., • Escalates material or staffing resource deficits to appropriate management., • Demonstrates patient-focused and cost-effective approaches to patient care resources., • Documents accurately and thoroughly in compliance with hospital, regulatory, and legal requirements and standards of care., • Facilitates patient throughput on shift basis., • Contributes to environment of care supporting clinical colleagues, patients, families, and healthcare team members., • Advances clinical competence in nursing practice from novice to expert., • Projects professional image and communicates with caring, respect, compassion, and empathy., • Incorporates ethical principles into decision making for patient and family., • Encourages and demonstrates scholarship, inquiry, lifelong learning, and innovation., • Acquires knowledge and skills relative to role, patient population, clinical specialty, and health community needs., • Participates in shared decision making through specialty practice councils, nursing department committees, and unit initiatives.Job Requirements, • NYS RN License required., • BLS certification required., • CRRN Med/Surg certification required., • Minimum 2 years rehab and med/surg experience required., • Minimum 2 years RN experience required., • 1 year travel experience required for Travel positions (not required for Per Diem positions)., • EPIC experience highly preferred.Schedule Information, • 3x12 hour shifts schedule., • Weekends required.Unit Specific Information, • Med Surg/Rehab Unit, • Average Unit Census = 18, • 24 Beds in Unit, • Staffing Guidelines = 1:6Additional Details, • Med Surg/Rehab Unit - Mount Sinai West, • Average Unit Census = 18, • 24 Beds in Unit, • Staffing Guidelines = 1:6
