Job Description General Responsibilities: Salary Range: $40.00 - $42.00 Hourly Schedule: Thursday through Monday with alternating weekends Plan, organize and manage with oversight of the RN Wellness Director all clinical operational aspects of the Reflections program by ensuring the highest degree of dementia care is provided to the residents through the implementation of the INVEST mission and the Brandywine standards, dementia training, resident care, and overall clinical operations for memory care. In addition, ensuring all applicable federal and state guidelines and Brandywine policies and procedures are met. Standard Requirements: 1. Supports the Mission, Values and Vision of Brandywine Living and the facility., 2. Is knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment., 3. Supports, cooperates with, and implements specific procedures and programs for: a. Safety, including universal precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs. b. Confidentiality of all data, including resident, employee and operations data. c. Quality Assurance and compliance with all regulatory requirements. d. Compliance with current law and policy to provide a work environment free from sexual harassment and all illegal and discriminatory behavior. 4. Supports and participates in common teamwork: a. Cooperates and works together with all co-workers; plan and complete job duties with minimal supervisory direction, including appropriate judgment. b. Uses tactful, appropriate communications in sensitive and emotional situations. c. Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems and concerns. d. Promotes positive public relations with residents, family members and guests. e. Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned. Essential Duties:  Coordinates recruitment and hiring process specifically for the Reflections Care Management team members  Completes staff performance appraisals/disciplinary action  Facilitates general orientation for all staff on Reflections Program  Conducts monthly in services on various dementia related topics for all staff  Responsible for environmental rounds to check for safety of exit doors, resident room safety and unit safety, and adherence to brand standards  Provides direct support to care management staff for behavior symptom management/resident care approaches  Coordinates and facilitates the RCP Training program for primary staff on unit. Responsible for tracking completion and scheduling of RCP classes.  Suggests residents for restorative potential and assures program is consistently carried out by staff to maintain resident's maximum practicable level of function.  Cooperate with other resident services for complete evaluation of care.  Assist in establishing linkages with and arranging for services from public and private sources for income, health, mental health and social services.  Assist the resident in making application for, and maintaining, income entitlements and public benefits.  Coordinate care planning meets per state regulations.  Coordinates, implements, and monitors all resident service plans with the Wellness Director.  Utilizes Brandywine standards and standards of professional organizations in conducting resident care.  Demonstrates knowledge and skills necessary to safely provide care appropriate to age of residents managed on unit.  Cooperates with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained.  Assist with Admits, transfers, and discharge residents as required.  Reports to the RN to assess and documents resident and family response to interventions.  Provides written and/or oral status reports of residents to assure necessary follow up actions.  Requisitions and arranges for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures.  Assists the physician on rounds and ensures that pertinent information is communicated to and from him/her.  Assists in resident rehabilitation activities. Where required, assists in ambulation, and provides emotional support. Monitors the results of physical therapy activities.  Gives direct physical and psychological care in emergencies, as well as in the presence of illness or disability in order to maintain life, provide comfort, reduce stress, and enhance the resident's ability to cope. Reports all observations and changes in condition to the RN.  Prepares and administers medications as ordered by the physician and utilizing all standards/policies of medication administration.  Ensures that adequate supplies and equipment is on hand to meet the nursing needs of the residents.  Complies with Controlled Substances policy and procedures in maintaining accurate inventories and controlled logs. Follows appropriate guidelines for investigation of discrepancies.  Reviews medication administration records for completeness of information, accuracy in the ECP of the physician's order, and adherence to stop order policies. Reports any discrepancy to the RN.  Disposes of drug and controlled substances as required, and in accordance with established procedures.  Monitors residents ,reports to the RN and take appropriate action as instructed by the RN and MD for side effects and adverse reactions to medications.  Adheres at all infection control policy and procedures.  Administers and documents all treatments as ordered and in accordance with policies and procedures.  Maintains accurate resident care records and document pertinent data reflecting the use of the nursing process, the care provided and resident response to care. Reports all changes in resident care or condition to the RN.  Assists in the development of resident care assignment sheets.  Reviews all resident care manager assignments for completion  Works in partnership with the INVEST Coordinator to ensure a therapeutic and socially engaging environment  Ensures safety of unit through environmental rounds Task List—(If necessary for position) Other Duties Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances. Reports all needs for assessment and change in resident condition or care to the RN. Other duties as directed by supervisor Qualifications Education/Training: Licensed Practical Nurse Proof of successful graduation from accredited Nursing Program and possession of an unencumbered LPN license. Training in dementia care desired. Experience: Previous experience with Dementia population preferred. Previous experience in a hospital, assisted living, skilled nursing care or other related medical community required. Working Conditions: Moves intermittently during working hours and subject to frequent interruptions. May be subject to hostile and emotionally upset residents, family members, personnel and visitors. Must be able to cope with mental and emotional stress of the position. Physical and Sensory Requirements: Proficient reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; auditory and visual skills • Knowledge of federal and state regulations, and of nursing practices, techniques and methods applied to health and wellness resident, • While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell., • An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
