CommonSpirit may ultimately pay more or less than the posted range as permitted by law. Job Summary and Responsibilities As a Hospice RN, you will: • Be a clinical leader. Apply your expertise directly in a patient's most comfortable environment, delivering high-quality, personalized nursing care., • Craft individualized care. Develop, implement, and dynamically adjust comprehensive care plans that promote patient independence and well-being., • Collaborate and coordinate. Work seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care., • Empower and educate. Serve as a vital resource for patients and their families, providing crucial teaching and emotional support., • Mentor and guide. Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team. Qualifications: • Graduate of an accredited nursing program with an active RN license in the state served., • Minimum of 1 year of nursing experience (Home Health/Hospice experience is a significant plus!)., • Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care., • Current CPR/BLS certification.
