Job Description Basic Function: To initiate, facilitate, and perform a variety of casework related functions pertaining to the development and implementation of the integrated, person -centered plan which encompasses all aspects of the participant’s life. The Case Manager/QIDP is responsible for ensuring each person served receives direct and indirect support across every aspect of their daily lives including dignity, self-advocacy, and increased autonomy. The Case Manager/QIDP is responsible for direct oversight and case management of individual cases. Reporting Relationship: Reports To: Director of Case Management Supervises (Position Title): N/A Principal Duties/Responsibilities: The Case Manager/QIDP is the primary internal, service provider advocate for participants on their caseload in compliance with agency mission, program guidelines, best practices for individuals with ID/DD, applicable laws, rules/regulations, and policies and procedures promulgated through code agencies responsible for surveillance and regulatory oversight of Ada S McKinley Program. The Case Manager/QIDP coordinates and leads the multidisciplinary team (ISC/ISSA, medical, clinical & behavioral, home & day program management, parent/guardian) through the collaboration process to meet the needs of individual’s served including but not limited to: design, implementation, and monitoring of implementation strategies used to meet quality of life outcomes indicated in person centered plan(s), completion of required assessments and required paperwork, oversight of the delivery of services provided across day and home environments, and evaluating whether services provided and recommended continue to be appropriate across settings. Person Centered Planning, Advocacy & Implementation • Serves as primary service provider advocate to participants including services and supports needed to help the person achieve their goals, promotion of and teaching self-advocacy, assuring individual(s) participate in all decisions affecting his/her life to the greatest extent possible., • Serves as primary contact person for guardians, interdisciplinary team members, and all outside service providers when applicable., • Collaborates with the interdisciplinary team (IDT) in developing and implementing the following:, • Ensures all interdisciplinary team members are invited to all meetings, including but not limited to:, • 30-Day Person Centered Plan (PCP) for new admissions, annual PCP review, any subsequent IDT meeting needed prior to annual PCP review, hospitalization/rehabilitation discharge planning, Ada S McKinley discharge., • Completes implementation strategies for outcomes written by ISC within 20 days of receiving parent/guardian approved PCPs and documents implementation strategy approval attempts., • Participates in communication, collaboration, and team meetings with all centralized support teams to ensure the coordination of service delivery, assurance of program quality, and integrate subsequent objectives/outcomes into the person- centered plan., • The Case Manager/QIDP maintains overall accountability for initial and ongoing training of implementation strategies for participants on their assigned caseload, essential duties include:, • Ensuring implementation strategy objectives written are specific, measurable, achievable, realistic, and time orientated., • Providing clear and concise, step-by-step instructions for implementation by direct care staff., • QIDP will review objectives/outcomes at least monthly and adjust implementation strategies accordingly., • Changes will be forwarded to the CDS Director of Case Management who will provide retraining as necessary. Programming & Quality Assurance • The Case Manager/QIDP maintains accountability for the upkeep of participant files on their caseload., • Completion of required paperwork, including but not limited to annual consents/releases and subsequent medical provider consents/releases, implementation strategies across home/day program(s), medical and psychiatric provider reports/appointment paperwork as needed., • Ensuring participant demographic information (state identification, social security card, current and emergency contacts, and face sheet photo), current psychological (within 10 years), annual person-centered plan & discover tool, & behavior service plan (annual) are present in the clinical file., • Completion of monthly Q note detailing participant progress toward achieving outcomes/objectives, community integration, summary of monthly services provided, assessment of appropriateness of services provided, and medical appointments attended. The Case Manager/QIDP is also responsible for providing this information to the respective ISC agency as well as parent/guardian., • Completion of DHS related assessment & paperwork that assures continuous appropriate placement including but not limited to annual ICAP, and STAR form as needed., • Documentation of collateral and direct participant contacts via appropriate progress note(s)., • Participation in internal and external file review, upon request & in compliance with HIPAA Inter-Department Collaboration, Leadership & Values The Case Manager/QIDP consistently displays professional and collaborative behavior in accordance with Ada S McKinley vision, mission, and values, and agency policies and procedures. The ideal candidate demonstrates leadership skills, initiates task and projects, takes ownership for their own professional and personal development, seeks opportunities for self and professional development, and communicates effectively with all Ada S McKinley persons served and staff in a respectful manner. • Upon request, the Case Manager/QIDP produces files and paperwork requested by internal & external department team members as relevant to the interdisciplinary team within a reasonable amount of time., • Where applicable, the Case Manager/QIDP assists members of the IDT team in obtaining additional documentation from parents/guardians and ISC personnel., • Where applicable, the Case Manager/QIDP works collaboratively with the Director of Performance, Compliance & QA to provide documentation required for intake, billing, DDD program eligibility & transfer., • Successfully completes Ada S McKinley’s required training, re-training, and any additional training as required by Ada S McKinley policies and practices, or as assigned by his or her immediate supervisor. Actively participates in any professional development opportunities, as appropriate., • Maintains annual American Heart Association or Red Cross certification in First Aid and CPR, • Attends and actively participates in department meetings and other meetings as required by supervisors., • Required to attend occasional overnight stays for conferences. Requirement: • Position requires a bachelor's degree in human services field, including but not limited to sociology, special education, rehabilitation counseling., • Must be Qualified Intellectual Disabilities Professional (QIDP) eligible or certified. Must have excellent interpersonal skills., • Must have at least one year of experience working with persons with intellectual and/or developmental disabilities., • Willingness to foster agency, department and program wide cooperation and teamwork through use of positive/constructive communication techniques., • Able to facilitate a positive, integrated, cohesive and supportive person-centered approach and attitude among all program personnel., • Have a strong passion for working with and helping adults with special needs., • Strong organizational skills and ability to manage multiple tasks., • Demonstrated verbal and written communication skills., • Proficient computer skills and mastery of Microsoft Office applications. Mental/Physical Demands: Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Have the ability to deal with problems involving several concrete variables in standardized situations. Ability to read and comprehend instructions, correspondence, and memos. Ability to write correspondence. While performing the duties of this job, the employee is regularly required to stand and talk or hear. The employee frequently is required to walk and use hands to finger, handle, or feel; and reach with hands and arms. The employee must frequently lift and/or move up to 100 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus. Environmental Conditions: The noise level in the work environment is usually moderate; work is performed primarily inside, but work requires occasional exposure to outside weather conditions. Occasionally exposed to moving machinery parts i.e. floor cleaning equipment, shrink wrap tunnel and to fumes from cleaning products.
