Job DescriptionDescription: LOCATION: This role is remote, but the selected candidate must live in the Raleigh–Durham (Triangle) area to support local activities and in-person needs (2-3 times a month). OVERVIEW: Reporting to the Network Director, the Care Coordinator engages in direct client care coordination, managing referrals and assistance requests within the market. This role is central to advancing the organization’s mission to serve Service Members, Veterans, and their Families (SMVF) by identifying individuals at risk for suicide and collaborating with teammates and partners to ensure timely, coordinated, and effective crisis response when needed. The Care Coordinator supports proactive, upstream engagement by facilitating early and appropriate connections to community-based resources that promote holistic wellbeing and long-term stability. Through collaboration with internal teams and external partners, this role strengthens referral pathways, enhances care coordination processes, and contributes to the continuous improvement of service delivery systems. In alignment with organizational values, the Care Coordinator fosters trusted, person-centered relationships and helps ensure SMVF receive compassionate, equitable, and high-quality support that promotes resilience, wellbeing, and improved outcomes. PRIMARY RESPONSIBILITIES: • Build welcoming, supportive relationships with Service Members, Veterans, and their Families (SMVF) by engaging in meaningful conversations about their military background, overall well-being, and personal goals., • Provide person-centered support across key Social Determinants of Health (SDoH) - Healthcare, Housing, Education, Employment, Benefits, and Social Enrichment, • Review assigned SMVF referrals and conduct thorough intake assessments, including suicide risk screening using the Columbia-Suicide Severity Rating Scale (C-SSRS), to prioritize service delivery., • Facilitate collaboration with direct service providers and support SMVF in connecting eligible and appropriate resources as needed., • Develop trust and create positive working relationships with team and community partners/providers/organizations within the market., • Maintain up to date knowledge on eligibility criteria, capacity, location, and referral protocols of local, state, and regional SDoH Key Providers and Key Programs in coordination with the Network Director., • Collaborate with Care Coordination teammates and/or Care Navigators, as needed, to coordinate care, facilitate comprehensive assessments with specialized providers, and connect SMVF to emergency crisis resources when appropriate., • Promote the health and wellbeing of SMVF by facilitating timely, accurate, and secure referrals to trusted community providers., • Reinforce best practices of military cultural competency., • Ensure eligible veterans are successfully connected to the VA (U.S. Department of Veterans Affairs)., • Document all SMVF interactions in the Organization’s technology platforms adhering to standardized procedures and following best practice guidelines., • Utilize dashboards, case conferencing, and supervision to effectively triage, prioritize, and manage cases effectively, supporting high-quality care coordination and outcomes for SMVF., • Complete required training in Mental Health First Aid and suicide prevention and effectively apply evidence-based tools and technology to document, track, and measure service delivery in collaboration with network partners., • Attend and participate in community engagement events alongside market teams and strategic partners to identify SMVF, increase awareness of available resources, and promote overall health and wellbeing of SMVF., • Demonstrate professionalism by consistently upholding organizational standards and embodying the values of Veterans Bridge Home, contributing positively to organizational culture. Requirements: QUALIFICATIONS: To perform this job successfully, an individual should meet the following criteria. Reasonable accommodation may be provided to enable otherwise qualified individuals with disabilities to perform essential functions. • 2+ years of professional experience in Case management, human services, and/or customer service, sales or equivalent demonstrated through one or a combination of the following: work experience, training, military experience, education (Social Sciences education preferred)., • Veteran or military spouse preferred or equivalent understanding of military cultural competency., • Preferred experience in handling and documenting Protected Health Information (PHI) and Personally Identifiable Information (PII) within electronic health record systems in compliance with HIPAA regulations., • Ability to identify potential risk and take appropriate action to prevent and/or intervene during crisis., • Highly organized with the ability to manage a large and dynamic workload, adapt to shifting priorities, and effectively manage time and resources., • Demonstrate commitment to ethical practice, cross-cultural awareness, and alignment with the mission and values of Veterans Bridge Home., • Excellent written and verbal communication skills, briefing and presentation skills for both in-person and virtual environments., • Demonstrated initiative, intellectual curiosity, and strategic thinking with strong attention to detail and the ability to work independently and autonomously, exercising sound judgment to manage responsibilities and navigate complex or ambiguous situations under pressure., • Ability to effectively communicate up, down, and across the market structure to meet the needs of the SMVF., • Exhibit a strong presence, outstanding interpersonal skills, excellent leadership and influencing skills, along with self-confidence, energy, and enthusiasm., • Excellent interpersonal and relationship management skills, with the ability to engage professionally and effectively with a wide range of stakeholders., • Ability to work collaboratively across diverse health and human services settings., • Demonstrated ability to set goals and clearly articulate the value and impact of work, activities, and relationships, • Proficiency using Microsoft Office Suite, Teams and SharePoint, and web-based applications; ability to learn new software systems utilized by VBH. SALARY AND BENEFITS: • Full Time Hours: Schedule will be determined in conjunction with the position supervisor to best support volume, community events and client schedules.
